Employee was cutting new dog bone mechanism bolts with the plasma cutter with cutting glasses. IE decided to switch to a welding hood but couldn't see the lines on the weld setting so IE switched to grind setting. IE woke us the next morning with eyes burning from flash burn. IE received prescription eye ointment.

IE was roof bolting when EE grabbed the wrong lever closing the jaws around EE's right hand causing a fracture to fourth metacarpal shaft fracture.

While roof bolting the IE was attempting to move the roof bolter cable out of the way while IE was installing a bacon strip. When the IE stepped back down onto the platform IE snagged the mast feed cylinder with IE's clothing causing the mast to move down pinching IE's right arm between the mast and roof bolt plate tray.

IE was grading out of the tail-gate when a roof bolt was kicked out by the lump breaker striking the IE above the left eye causing a laceration that required stitches to close the wound. It also caused a tooth to be chipped.

I.E was trying to straighten frame work on the tailgate drive with a crib block between the anti slew bracket and tailgate drive frame. I.E was holding the crib block while another employee was operating the shields pulling the pan-line back. The crib block popped out striking I.E in the face knocking I.E unconscious and causing a fracture to zygomatic bone.

While roof bolting the employee's drill steels got stuck in the roof mesh. The roof bolting partner set angle bolt causing the roof mesh to move freeing the steels from the mesh. The steels came out of the roof striking the employee's right pinky finger that was on top of the jaws causing a wound that required stitches. Right pinky finger also has a small fracture.

The employee was setting a shield on the new long wall face. Before the shield could be set, the side shield needed to be pulled in. EE was kneeling down and had his hand in wrong position. Another employee closed the side shield after giving warning and caught EE's hand between the shield and side shield.

THE EMPLOYEE RECEIVED 7 STITCHES TO CLOSE THE LACER-ATION AND RETURNED TO WORK THE NEXT DAY REGULAR DUTY.
