Excavator operator was cleaning along the high wall when a large piece of rock was dislodged and rolled into the cab pinning the operator in the cab for a period of 2 hrs and 47 minutes. Victim suffered a serious injury to the neck.

A DRILL STEEL CAME LOOSE AT THE TOP OF THE CAROOSEL AND WHEN TWO EE ATTEMPTED TO PUT IT BACK ANOTHER STEEL CAME LOOSE AND STRUCK HIM ON THE LEFT WRIST.
