Employee was operating a Cat D11N dozer pushing rock towards a Komatsu loader when the highwall fell on the right side and partially covered up the machine. The operator was entrapped a short period of time and extracated himself by kicking out the window glass on the oposite door.

Excavator operator was cleaning along the high wall when a large piece of rock was dislodged and rolled into the cab pinning the operator in the cab for a period of 2 hrs and 47 minutes. Victim suffered a serious injury to the neck.

EE HAD MADE REPAIRS TO A HEATER MOTOR A 992 C LOADER. HE TURNED THE MOTOR ON AND COULD NOT TELL IT IT WAS RUNNING SO HE STUCK HIS HAND IN THE AREA OF THE FAN WHICH CAUSED LACERATION TO HIS RIG HT HAND.
