Changing cutting blades on W-900 loader when the bolt started to spin EE put EE's hand on the bolt to push it back up in the socket while the bolt was turning and this resulted in a cut to EE's palm.
Mechanic was installing belly pan on a D-10 dozer. While installing a bolt using an impact wrench, resistance was felt before bolt was seated so the bolt was backed out. When mechanic lowered wrench the bolt dropped down and hit EE's lip causing a split on the inside. One stitch was required as a result of the incident.
EE was using a high torque on the D11 performing final torque. He was doing final torque on segment bolts doing 120 deg turn when reaction arm pinched the tip of his right index finger between reaction arm & hub. This pinched & severed the end tip of his right hand index finger.
A chunk of material came off of the shovel bucket and landed on the headache rack of the haul truck. Driver later complained of neck pain. Seen in the ER and was diagnosed with a muscle strain, placed on restricted duty.
Grader drove over a large rock at the edge of a cut. Operator complained of neck and back pain. Transported to hospital for evaluation of back and neck strain, began missing time on 10/03/06.

Employee states that while using a torch to cut mud flap bolts from bottom of truck bed. Employee twisted and felt pain in his lower back. He was seen by a physican who diagnosed him with a muscle strain and placed him on restricted duty.
THE SMALL RENTAL HOE TIPPED ONTO ITS SIDE AND THE OPERATOR'S LEFT FOOT WAS CAUGHT UNDER THE SIDE OF THE HOE CAUSING A BRUISE AND SPRAIN. HE WAS PLACED ON RESTRICTED DUTY.
EE WAS USING A CUT OFF SAW TO CUT A PIECE OF 2X2X1/2" ANGLE IRON. THE EE HAD HIS HAND ON THE ANGLE IRON AND WHEN THE CUT WAS CLOSE TO COMPLETE, THE ANGLE IRON ROLLED TOWARDS THE FENCE/BACKGUAR D AND PINCHED FINGERS BETWEEN ANGLE IRON. AND BACKGUARD. THIS RESULTED IS LEFT HAND FINGER TIP FRACTURES 3RD AND 4TH DIGITS CUT OFF SAW.
OPERATOR ALLEGES WHILE OPERATING DOZER HE STRUCK A ROCK AND JARRED HIS BACK. HE BEGAN TO NOTICE PAIN IN LOW BACK LATER IN THE SHIFT.
EE ALLEGES CUTTING OFF BOTTOM OF PLATE ON A BUCKET & PLATE STRUCK EE ON THE HEAD. TCM WAS MADE AWARE OF THIS ALLEGED ACCIDENT ON 3-26-02.
EE WAS GRINDING THE WELD ON THE 1210 RIGHT WING SHROUD OUTSIDE THE REPAIR AREA. THE 9" HAND-HELD GRINDER HE WAS USING HIT A RAISED AREA & DEFLECTED BACK INTO HIS LEFT KNEE.
EE MAY HAVE GOTTEN MATERIAL IN HIS R EYE DURING THE COURSE OF HIS SHIFT. EYE MEDICATION.
