Miner got a foot caught between a push plate and stop block resulting in a fractured first and second toe on left foot.

Employee was trimming the front wallplate with a battery powered saw and while making a cut, the saw slipped causing a laceration to employee's thigh. Employee received 8 sutures.

EE received a left hand laceration when EE reached out to steady a chop saw. The blade was still turning after a very recent cut.

EE was installing a 18" split set with a Jackleg. As the EE was pulling the jackleg out of the bolt, the jackleg pivoted and pinched EE's hand between the jackleg and the wingnut.

EE was cutting a handrail with a battery powered porta-band. As the saw cut through the rail, it contacted their left index finger resulting in a laceration.

EE was struck in the face by a rock that was dislodged from the face. The miner was installing a bolt into the fingers of the bolter when the bolt hit the face, a rock slide down the bolt and struck the employee.

While bolting a piece of rock fell through the chain link fencing and struck the employee in the arm.

EE Was removing the seals on a Wilson-Snyder pump with a 1 ton overhead hoist when the hoist fell from the hook and struck the employee on the hand.

EE received a laceration to left knee when a rock they were attempting to dislodge rolled down the rib, hit a small ledge and struck them in the knee.

While installing ground support, the employee got finger caught between the machine and the leg.

EE gets middle and ring fingers pinched between traverse slide and rotation slide cylinder nut of drill after resting hand on the side of the traverse slide not realizing EE had put two fingers in the opening for the rotation slide cylinder.

The Operator applied air to the jackleg's feed leg too quickly while driving a rock bolt, EE lost control of the machine and lost EE's footing, IE fell with the machine in hand to the ground, resulting in EE's left index finger getting pinched between the jackleg and the feed leg.

Employee was setting up to shoot a shotcrete wall and was struck in the elbow area by the trans-mixer that had the mixed shotcrete. Later received notification that the employee had to receive physical therapy making this injury reportable. Due to COVID quarantine requirements, employee is unable to come to work.

EE was using an impact wrench with a socket on it to tighten a nut on a yoke. The socket came off while in use and struck the EE in the mouth.

Employee was operating a 2 yard mucker in a sill. Operator pinned their right foot between the rib and the mucker resulting in lacerations and fractured toes.

Employee was pulling core tube when finger got pinched between stuffing box and wireline.

While jackleg bolting in the 5650 JN the machine was over head, EE un-chucked the steel and lost control of the machine. When the machine broke loose it pinched EE's thumb in the pinch point, smashing it.

Employee was struck on the top of right hand by a roof bolt resulting in a laceration.

While installing a water pipe, EE accidently opened an air valve causing an injection injury

Drill rod fell from vertical hole pinching index finger between rod and drill hammer.

EE's left foot (small toe) was crushed between the hydraulic outrigger jack and ground while repairing a part on a powder truck near the rear outrigger.

EE was bolting up a hole in the wire in the back at the intersection, EE said they drilled by either a bolt or through a slip. When EE pulled the machine back out of the hole it hung up. When EE got it to let go the leg was released all the way and the machine came down all at once.

EE was drilling when a rock contacted eye while wearing safety glasses resulting in a prescription to treat a scratch to eye.

EE and partner were using a jackleg drill for installation of rock bolts. While pulling hard on the jackleg to extract the drill steel from the hole the steel suddenly pulled free causing EE to fall backwards and right thumb got caught between the jackleg and a concrete slab causing crushing injuries to the thumb.

Employee received lacerations to chin from component of an air powered pump.

The employee was bolting the lower hole on the right rib with a Jackleg drill, when the employee started to pull the machine out of the hole he felt a pain in his lower back. The employee received medical treatment on 7/24/2015.

Employee hit nose inside the cab of an abruptly stopped vehicle.

On 9-15-14 E was pushing a 12' dywidag up into a hole drilled in the back when a piece of material fell behind his safety glasses and got into his right eye. E flushed the eye repeatedly. When E returned to work on 9-16-14 he was taken to the eye doctor where a piece of embedded metal was removed from his eye.

EE was burned on his right bicep. EE had been bolting with a 4' split set driver and leaned it against his bicep while it was extremely hot from driving in split sets.

Equipment operator was trimming the face, back, and ribs with an excavator with a road header attachment. He finished trimming and was starting to tram back out. Operator traveled 4 feet and went over uneven floor causing his left arm to activate the swing arm lever to the left. The road header swung to the left striking the injured employee.

EE was installing rock bolts in rib and debris got into his eye. He worked through his entire shift. When waking up the next morning, he immediately noticed that the debris did not wash out, went to his eye doctor and had it removed, and reported the incident at the corporate office.

EE was drilling with jackleg drill when the steel shanked and caused his right hand to hit the D-Handle of the machine.

Employee injured his left shoulder when the drill steel he was drilling with hit an existing bolt causing the jackleg to spin.

The driller was assisting his helper to lower a tube. Driller slipped and stuck his hand out to catch himself. He caught the ring finger on his left hand. It cut just a little less than a quarter inch of the finger off.

EE assumed the operation of the jackleg drilled out for and installed four bolts then moved over to his left to begin the next row. He drilled three bolt holes and was on the 4th hole, which was about chest level, when the goose neck came loose striking him in the nose.

Employee was tasked to weld. Employee misused a welding helmet. The helmet was for use with a hard had (required clips). The employee did not use it with a had had and instead held the helmet in place with one hand, thus making the equipment ineffective and causing flash burns.

Employee was attempting to adjust the split set in the carousel, when the plate slipped down causing a small laceration to the lower right wrist area resulting in three sutures.

EMPLOYEE WAS WORKING ON THE TRACTOR AND AS HE WENT TO GET DOWN OFF IT, HIS PANT LEG CAUGHT THE CLUTCH PEDDLE CAUSING HIM TO LOSE BALANCE AND FALL. HIS LEG HIT THE FRAME OF THE TRACTOR CAUSING A SMALL FRACTURE TO HIS LOWER RIGHT LEG

Employee was bolting in a stope using a jackleg, when he attempted to pull the jackleg and steel out of a hole he just drilled he injured his shoulder.

Employee was operating a mucker when the bucket caught the rib and bounced him into the overhead air conditioner unit. He struck his head. He completed the shift without pain and went home. He woke up the next day with a stiff neck and was given days off work

Employee was installing new doors in the explosives storage area underground. As the door was being moved into place with a boom crane, the door hung up and the employee attempted to get it loose. It swung towards him striking the back of his hand and causing a deep laceration that required sutures.

Workers were rotating the slide on a long hole drill. As the slide rotated, it struck the drill steel. The drill steel shifted suddenly, striking the worker in the mouth.

Worker lost his balance while drilling with a jackleg. The drill began to fall and he stopped it by grabbing the saddle/hammer area. The drill rotated back on the leg, pinching his right hand.

Welder was using an air arc torch to remove liners from a truck. He finished up & removed his welding PPE. He was walking over to office to use radio when he saw the ground clamp for air arc and decided to remove it so he wouldn't forget to before he moved the truck. The clamp was still hot from cutting job resulting in Burns to the right hand palm.

Employee unintentionally depressed the air actuated trigger on a grease gun. Discharge from the grease gun got into the employee's right eye. The eye was flushed with eye wash underground and he was then taken to an optometrist. The optometrist prescribed an antibiotic to prevent an infection.

While removing compressor rope, using a sheave pulley that was with original compressor setup, EE was stationed outside the pinch points and was the signal person for the operator on the Mini Excavator. The injured employee is not sure how exactly he ended up with hand into the pulley. His left hand, index, middle and ring fingers were injured.

Employee was kneeling while using a hand held 9" grinder . The grinder jumped to the left and caught him in the leg causing an approximately 2" x 1/4" laceration on his left leg.

A Mechanic was helping to finish the tire job on a haul truck, he was using a torque wrench on the right front wheel and felt a sharp pain in his left lower abdomen or upper groin area. He checked himself and saw a "bulge" in the lower abdomen area. The mechanic was transported to hospital for treatment.

While bolting the back, the jackleg the miner was using, hung up and the drill spun around and the handle struck the miner in the mouth. The laceration required 8 stitches.

Miner was bolting back with a jackleg drill. The leg on the drill blew apart and the jackleg drill struck the miner on the right ankle. The miner was placed on light duty.
