Showing all 5Employee was grinding a 1/2" piece of metal. The metal was clamped in a vice. The grinder bound up or caught on something causing it to jump back, hitting the employee on the left wrist resulting in a laceration.
Cutting 1" channel off of the top of the portable impact crusher hood.
Employee was removing plates from CHX-1 (carbonate heat exchanger) with a pneumatic impact wrench. The wrench slipped & he tried to catch it with his free hand. A retaining wire on the socket caught his hand causing lacerations to right palm. He was taken to local hospital where he received 8 stitches.
EE WAS SHEARING A PIECE OF 1/4 INCH STAINLESS STEEL PLATE FOR THE LAB. AS HE WAS SHEARING OFF A CORNER OF THE PLATE, THE FOOT ON THE SHEAR MACHINE CAME DOWN TO HOLD THE PLATE. HIS RIGHT THUMB WAS CAUGHT BETWEEN THE FOOT AND THE PLATE, AMPUTATING THE TIP OF HIS RIGHT THUMB APPROXIMATELY 1/2-3/4 INCHES.
