EE had been using a die grinder throughout the day, later in the day noticed an irritation in his left eye. Metal shaving was found in his eye the next day after reporting to the onsite medical facility to have it examined. Employee received antibiotic drops for treatment after the metal shaving was removed.

While installing roof bolt,the employee reached to grab the steel an put his other hand on the top of the drill box, other steel came out of the roof landing and landed on his hand smashing his finger between the steel and the box. Employee was transferred to ER and received stitches. Returned to work with no restrictions.

On 1/24 a.m. EE had been welding/cutting/grinding on sections of the ore dist. building installing unistrat. He was wearing all required PPE but at some point around 9:00 a.m. he felt a foreign object in his eye. Assuming it was dust he attempted to wash it out, when that failed he notified the safety dept.

Mechanic was welding a baffle plate inside Mono 1 dissolver, while making an up hill weld the mechanic came into contact with a sharp edge of the baffle cutting his right index finger. The mechanic was sent to Medcor where his finger was evaluated and the laceration was medically glued.

Employee was using a 1/2 inch drill to drill a hole in sheet metal. The hole saw grabbed, causing the drill to spin out of his hand. The drill had a side handle attached and the twisting of the drill caused the side handle to strain his left thumb. Injury became reportable when the employee had surgical intervention on 7/19/2011.

Cutting a slip on flange off of a 4" pipe with a metabo cutting machine to make a modification to the pipe spool. The cutting wheel came in contact with his right index finger.

The shift started the previous night at 9:00 pm, we were abrasive grit blasting in unit 7 precipitator. They crew had moved down to the galleries and had begun blasting. One sandblaster did not see the other sandblaster on the other side of the gallery and blasted him. He recieved abrasions on his stomach and forearm.

Using a metabo to cut 1/2" piping loop from chiller. Two of the connections were cut & was starting to cut the third while supporting the loop with one hand. Almost through pipe when tool bound up kicking out & striking employee in R leg above knee causing large laceration on R thigh.

Replacing bearings & shaft in ventilation fan. One person was turning fan to set clearance & other was looking for the area where blade was rubbing. Reached to touch area where he thought fan & housing were rubbing; his right thumb was cut when turning blade struck his thumb.

Employee was unbolting pipe flange with the use of a 3/4" impact gun. The socket broke & the sharp edge cut through his glove, lacerating his left thumb.

Employee was using a bit wedge as spacer with a porta power to push out bit block insert on the longwall shearer cutting drum. As he pressured up the porta power, the wedge came out hitting him in the palm of the right hand.

Employee running the dry bolter. While he was drilling a hole the drill steel plugged up; he was removing steel from the top; he did not notice that the steel was still slowly rotating. He grabbed the steel & it bent his right thumb back.

Employee was drilling hole thru wall & drill bit hung up in the metal causing the drill to twist in his right hand. Returned to work the next day with right thumb swollen. He was examined by a physician & it was determined that the thumb was actually broken.

The employee was jacking the jaws of a crusher closed with a porta-power. While applying pressure to the porta-power it slipped loose, flying out and striking the employee in the forehead causing a laceration that required 4 sutures.

Employee was operating the headgate end of the shearer; heading towards head when shearer threw rock about 14' in diameter. Employee was about 20' away when the rock bounced off the top of the spill tray & struck him on the left leg & right foot. Medical information not received until middle of March.

Employee was installing screen with a Fletcher roof bolter when the bolt hung up as it was being pushed into hole. The employee was holding the bolt and his hand was shoved into the mast, smashing his fight little finger, requiring the stitches.

While moving the longwall monorail with a winch, the chain securing the winch broke. The winch swung and hit the employee in the face above the right eye, requiring sutures. The report is being filed at this date because we have been waiting for additional medical information. We have not received the information, however they may be a possible skull facture.

Employee was working on new fletcher bolter drilling a gas hole. Auger got stuck so they dropped rotation motor & tried to remove auger from chuck. Didn't work so they raised auger back up & clamped centeralizer & lowered feed motor. Auger came loose from centeralizers & struck employee on right ring finger.

Shoveling out elevator full of product; product was 370F. Employee's feet would get hot and he would stop shoveling for awhile. Didn't notice anything except a red big toe until several days later when his feet started to hurt and he noticed blisters on little toes.

Employee was grinding holes in cable spools using a die grinder with a tungsten burr. Employee had on safety glasses and faceshield. Foreign body entered the eye and had to be extracted. (This accident was previously submitted but does not show up on the database. This may be a duplicate report.)

P-5512 was not pumping. Operator shut off pump using local start/stop station. He climbed down ladder & opened belt guard inspection door. He could not see very well because of poor lighting. He put his left hand on top of the drive belts. The pump was still turning which caused his left hand to be caught between the belt & pully & amputated the end of his left middle finger.

OPERATOR WAS INSPECTING #7 BOILER FOR SLAG. TO DO THIS SHE USED INSPECTION PORT (D) WEST. THE BOILER DRAFT WENT POSITIVE, CAUSING HIGH TEMPERATURE GASSES TO EXIT THE INSPECTION PORT. THIS RESU LTED IN A BURN TO HER LEFT ARM, AND SINGEING OF HER EYELASHES AND HER HAIR.

EE HAD TURNED DISCHARGE OF STEAM JET ON AND WAS WALKING AROUND SUMP WHEN HE STEPPED ON A PIECE OF GRATING. THE GRATING SLID OFF THE SHOULDER CAUSING EE AND GRATING TO FALL INTO THE TRENCH. EE BURNT HIS LEFT FOOT AND LOWER LEG, AS WELL ASA PATCH ON HIS RIGHT FOOT.

EE WAS STANDING BETWEEN TWO SUPPORTS FACING THE SHEAR APPROXIMATELY 3' AWAY. AS THE CUTTING DRUM WAS RAISED, ROOF SPARKS WERE SEEN. A ROOF BOLT PLATE WAS CONTACTED. A PIECE OF DEBRIS STRUCK EE IN RIGHT SIDE OF FACE, NEAR NOSE, BECOMING EMBEDDED IN THE SINUS. SURGERY WAS REQUIRED TO REMOVE IT.

2/29/00 EE REPORTED HE WAS CLEANING U-841 KREB POT W/A HOSE. THE HOSE TURNED TO STEAM, HE DROPPED THE HOSE BURNING BOTH LEGS. AT THE TIME OF THE INJURY EE WAS RETURNED TO WORK. ON 3/6/00 HIS B URNS BECAME A LOST WORK DAY.
