Miner was removing a handrail from a piece of equipment. The handrail was supported using an overhead hoist. The strap on the handrail came loose causing the handrail to shift and pinch the miner's finger causing a laceration. The miner was wearing gloves at the time of the incident.

The brick rig was in use in the kiln to support brick being installed. Pogo sticks are set on the rig to the brick to keep pressure on them until the brick ring can be keyed. The brick rig shifted causing the pogo sticks to lose tension resulting in the brick above to fall, severing the last 3/4inch of EE's finger tip.

Employee was welding and grinding on date of incident. EE was wearing proper PPE, face shield and safety glasses. At some point, EE got a couple pieces of metal in right eye. Didn't bother EE until after the shift. Metal could not be flushed and had to be scraped out.

Employee positioned a platform ladder along side a ventilation fan to clean with pressure washer. Water valve was opened and the pressure was more than expected knocking the ladder over sideways. Multiple contusions, right shoulder, arm, ribs and hip.

Ee was using a torch to burn off the bolts of the take up bearing on the tail pulley of chips cross belt of the Nordberg. He reached behind the bearing, while torch was still lit, to clear mud off of the bolts and his left wrist came in contact with the flame.

WHILE GRINDING, THE METAL PIECE LURCHED OFF THE WHEEL AND FORCED EMPLOY EE'S HAND TO GET CAUGHT BY THE WHEEL. EMPLOYEE'SLEFT THUMB SUFFERED AVULSION TYPE INJURY. ALL SAFETY ITEMS CHECKED ON MACHINE AND WERE FOUND INTACT.

COMPRESSED AIR WAS BEING USED TO DISLODGE ICE FROM A PIPELINE. AS THE ICE BECAME DISLODGED THE COMPRESSED AIR AND ICE WERE FORCED OUT OF THE END OF THE PIPE. THE COMPRESSED AIR EXPANDED AND TH E FORCE OF THE EXPANDING AIR KNOCKED THE INJURED OVER. INJURIES INCLUDED DISLOCATED SHOULDER, BROKEN CLAVICLE AND MINOR CUT.
