On September 1, 2020 the employee had a follow up visit with Doctor for a previous minor burn. On a follow up visit the burn had some infection and the doctor prescribed an antibiotic and two days off work. Then employee received a return to work with no restrictions on September 3rd.

A tire technician was removing the inside dual lug nuts of a Komatsu 100 ton truck (T331). While positioning the 1' spline gun the technician twisted wrong causing employee to strain employee's back below the left shoulder blade.

An experienced miner was blading an area. While working the second pass, EE leaned forward to get a better look at the material coming out from the blade. EE came in contact with a large, unseen, rock that was part of the fill material. In doing so the equipment came to an abrupt stop causing head to come forward and the tooth coming into contact with the steering wheel.

A haul truck was having trouble getting backed into the shovel for loading. To assist the truck into position due to a slight grade and loose material, the shovel operator put the bucket in the bed of the truck for weight/traction and in doing so the bed of the truck raised and then returned to its lowered position causing a jarring effect in the cab.

Employee was changing the tire of a Caterpillar 777 end dump using a hydraulic operated bead breaker to loosen the inner ring. After the ring was loosened he grabbed the bead breaker to keep it from falling to the ground and inadvertently placed his index right index finger between the slide rails while the hydraulic ram was in motion, resulting in smashing his finger.
