After using plasma cutter, maintenance personnel lifted their face shield dislodging metal dust onto their face. Some of the dust went behind their safety glasses and into their left eye. Metal debris was removed from eye and antibiotic drops prescribed.

Showing all 20Employee was placing paper bags on 50 lb. bagger spout when the bag on the spout to EE's right ruptured. The bag ruptured in such a way that the clay was directed towards the right side of the employee's head. This dislodged EE's safety glasses and got clay in EE's eye.
While taking the bolt out of the hammer tip in the mill. When turned on the impact wrench the bolt backed out and the head of the bolt cut through the glove and cut middle finger on left hand.
Employee placing pallet into palletizer. Pallet nail became caught on chain conveyor causing the pallet to stop suddenly and strain the employee's back.
Employee was manually installing roof bolts. Was in a man basket inserting mechanical roof bolts, using an impact to install bolts, and using torque wrench to tighten roof bolts per spec. Shoulder became sore in the process of setting approximately 80 roof bolts in a shift and was still painful 2 weeks later so employee reported incident on 8/16/2020.
Employee was working on changing the belts on separator fan on mill 4. They had locked everything out and tried. As they were removing the belts the fan rotated and smashed the right thumb and tore off the fingernail on the left thumb as well as the skin. It looks like with other equipment running airflow caused the fan to move.
The employee was working on a single drill cylinder holding valve and the hydraulic pressure behind it pushed the employee's finger into the main boom causing a laceration to the left finger. Employee received stiches in finger.
Employee was using a pneumatic impact wrench to repair equipment. When air was supplied to the wrench, the front facing exhaust stream of the wrench stirred up debris which was blown into the employee's eye.
Employee was grinding off a bearing and a piece of debris flew into left eye. Not first reported Friday because employee believed it was just a piece of stone, not steel. Employee was wearing safety glasses and grinding shield.
Employee ran a jack hammer to break up the dried up/hardened sludge that had built up on the floor of the mill on February 1, 2016. On February 9, 2016, employee reported to supervisor that employee had sought medical treatment the previous day for swelling in the groin area.
Maintenance mechanic lying on side while welding had small piece of hot metal fall into left ear. Some temporary hearing loss has been apparent, full damage unknown at this time.
Employee was cleaning out lumps of clay from a rotary valve; the employee stuck his hand in the rotary valve, while it was running, and lost the very tip of his third finger on his right hand.

Employee rested left hand on bagger frame crossarm. When hook ctrl arms were raised, his index finger was pinched between the crossarm and the metal part of the lifting arm. Employee works for a temporary agency at our facility. Since the temp agency does not have an ID#, I am reporting for them.
While lowering drill pipe to connect tool joint, helper put left hand under drill pipe being lowered to remove dirt. Middle finger was jammed between drill pipe causing laceration of finger. Helper was wearing gloves.
EE WAS REPLACING THE HOIST FRICTION BAND ON LARGE DRAGLINE #17. THE TASK REUIRES THAT THIS DEVICE BE HOISTED OVERHEAD AND MOVED TO OUTSIDE AREA. DURING THIS PROCESS, THE DEVICE FELL DUE TO INADEQUATE RIGGING. IT DEFLECTED OFF THE MAIN GEAR BOX AND STRUCK EE CAUSING A SPRAINED WRIST AND BRUISED RIBS
Employee was bagging product on bagger machine when a bag burst and spewed kaolin clay upward onto employee. The force of the material covered the employee's upper body resulting in some of the clay getting into his left eye. First aid was provided, but the eye became irritated the next day & medical treatment was needed.
ATH EMPLOYEE ENTERED DOCK #6 AREA TO LOCATE TRUCK DRIVER TO REPORT PROBLEM WITH HIS TRUCK. WHILEINFORMING THE DRIVER OF THE PROBLEM, A UWI EMPLOYEE WAS IN THE PROCESS OF RE-ENTERING THE TRUCK TO COMPLETE UNLOADING THE FINAL TWO PALLETS OF PRODUCT WHEN THE ATH EMPLOYEE WAS STRUCK ON THE RIGHT FRONT BY THE TOWMOTOR. NEITHER EMPLOYEE SAW THE OTHER UP TO THE TIME OF THE INCIDENT.
WHILE TRIMMING SLOPE OF MINE WALL A LUMP OF HARD DIRT CAME LOOSE UP ABOVE ROLLED DOWN SLOPE HIT EXCAVATOR BUCKET BURSTING WITH A PIECE RICOCHETTING INTO CAB'S WINDSHIELD PARTIALLY STRIKING OPE RATOR CHEST AND HEAD, RESULTING IN HIS HEAD AND CHEST AREA BEING BRUISED.
