EMPLOYEE STATED: The mechanic was drilling out rivets when a small piece of metal landed on the mechanic's cheek and subsequently slid under the mechanic's goggles. Mechanic was treated by a local eye doctor with a prescription.

EMPLOYEE STATED: The lab technician was preparing a lime sample for the pulverizing mill when EE cut EE's thumb on one of the mill's blades, which resulted in stitches.

The employee stated they injured right thumb while grinding a piece of metal using a bench grinder in the maintenance shop. The thumb came in contact with the metal that employee was grinding causing a laceration.

EMPLOYEE STATED THEY WERE cutting piece of 1 1/2 inch angle with a 4 inch die grinder when cutting disk caught on metal, kicked back and cut top of EE's right thumb. Employee was wearing proper PPE when issue occurred. EMPLOYEE RECEIVED 4 STITCHES AT OCCMED.

Employee stated employee was using drill and the bit caught the glove material while drilling on expansion joint. Bit pulled into skin causing cut on left hand palm. Employee went to ER, received 5 stitches.

Employee stated EE was standing behind an employee that was using a cutting torch and a piece of hot slag fell into EE's right eye. EE was taken to the eye doctor where the piece of slag was removed. Employee missed 3 days from work.

While unchoking a hopper with an air lance the employee stated he smashed his left ring finger between the air lance and the metal lip on top of the hopper. The employee received 3 sutures to his finger.

While cleaning out concrete footing with a backhoe the operator shifted the bucket to the left and boomed down. The edge of the bucket swung farther than expected and a tooth on the bucket struck the left foot of an employee that was directing the operator.

While cutting or grinding the employee stated he got a small sliver of metal in his left eye. The employee was sent to the eye doctor and the sliver was removed and a prescription medication was given to him. The employee further stated he was using goggles but a piece got behind them.

While using an air lance to clean out the pre-mixer on the hydrate mill, a small steel door fell on top of his fingers. As the door fell, he jerked his hand out, causing a small fracture and a laceration that required sutures.

EE was installing coal mill ductwork above the building roof line. The ee welded the duct to hold it into place in order to fit all the duct together. The foreman called for the duct to be released from the crane & at that time, the duct broke lose pinning ee between duct & roof beam. The ee was instructed on the installation of the ductwork.

EE was welding picking eyes on a rock box, when a piece of hot slag fell onto left glove, burning through glove and onto wedding band on left ring finger. 2nd degree burn around finger where wedding band was worn.

WHILE UNCLOGGING A PIPE WITH AN AIR LANCE HE SHOVED THE AIR LANCE INTO A PORT & GOT HIS RING FINGER STUCK BETWEEN THE AIR LANCE & THE PORT CAUSING A LACERATION. THE LACERATION REQUIRED SUTURES.

Walden's employees were water blasting the chambers of a vertical kiln when the water and material reacted within the kiln building pressure enough to blow the hot material out onto the employee. Employee had standard water blast PPE on.

EE STATED HE WAS USING AN AIR WRENCH WHEN HE SLIPPED OFF THE NUT, ACCIDENTALLY PULLED THE TRIGGER WHICH SPUN THE SWIVEL ATTACHMENT CAUSING THE KEEPER PIN TO STICK OUT. AS IT TURNED IT CUT HIS FOREARM. THE LACERATION HAD TO BE SUTURED.
