Operator was working on drain hatch for Zn stock tank. Operator had difficulties removing bolt. They then asked for help from another operator to cut bolt. They started cutting the bolt with pliers, as the cut was being made the sawzall blade jumped and hit the operator with the pliers in the hand causing a cut requiring stitches.

IP was using a cordless hole saw to drill penetration holes in the wall of the 6030 Powerhouse. While drilling the drill bit contacted a support beam on the other side panel. The contact resulted in the drill twisting the IP's hand. Fracture to 4th metacarpal.

DML mechanic while installing an axle on a concentrate trailer was tightening the u-bolts. The socket slipped off the u-bolt nut and 1" drive air gun struck the mechanic in the face. The mechanic received a broken tooth.

Foreign debris entered worker's left eye while grinding tube ends with an end grinder. No particle requiring removal, but scratched cornea and antibiotics given. Double eye protection was on and in use while grinding.

EE using a circular saw cutting metal strips. EE released the trigger, moved the saw down towards themselves. The blade was still in motion and cut through the EE's clothing resulting in a laceration to the left thigh.

DML Mechanic was under a truck installing a transmission with a 3/4" drive pneumatic impact gun. The impact socket slipped off the bolt head, came loose from the air gun, and fell down on the Mechanic's closed upper lip, chipping a tooth. The Mechanic completed shift, visited the Medic, and was returned to normal duties.

Workers were on the last hole of pipe support installation. While placing mag drill in location on bottom side of I beam, worker inadvertently pushed the start button of the drill. Drill wrapped up worker's leather cut glove and thumb around bit. The worker immediately pulled their hand from the rotating bit. Worker on the ground saw what took place and unplugged the drill

IP had just finished work for the day. IP stopped at the smoke area before heading in to camp. IP felt discomfort in left eye and it started watering. IP was seen by the onsite PA. PA diagnosed a rust ring, drilled the eye, prescribed antibiotic drops and released IP to work. IP had been grinding on pipe during shift.

EE was grinding and felt something enter EE's eye. EE continued working. At lunch EE flushed it. After lunch EE's eye was still irritated. Reported incident, went to clinic, PA drilled out an embedded foreign body. No rust ring present.

EE pulled down welding hood and felt something in EE's eye. EE flushed the eye and proceeded to finish EE's task. No other issue with the eye. The following day EE's eye was irritated when the sun came out. EE flushed it again. Around lunch time it was still irritated. EE informed the Supervisor and Safety escorted to on site PA. A rust ring developed requiring it to be drilled.

While moving a turbo charger rotor, Employee pinched finger between the rotor shaft and the crate they were setting the rotor in. Resulting in a laceration to the left ring finger.

Employee cutting 6" channel iron with portable band saw. Channel iron began to wobble as employee neared the end of the cut. Employee reached under saw to steady material and contacted the band saw blade.

Worker was using an electric drill to bore out a small pipe for the firing pressure tool. The drill snagged, and the drill spun in the worker's right hand. The drill handle spun and hit the worker's right pinky finger pushing the finger at an odd angle causing a fracture/dislocation of the right fifth finger/knuckle.

The drill crew was tripping out drill rods and laying them in the rod tray located on the side of the drill rig. The employee was using a wrench to raise the rod to loosen the rod hoisting bail. Once the hoisting bail was removed the employee pulled out the wrench causing the rod to shift and come in contact with EE's right hand.

A millwright's finger was pinched between a belt and sheave while rebuilding a pump. The pump was locked out and the pinch occurred when the employee manually turned the sheave wheel to thread the impeller. The end of the index finger was torn off and the tip of the finger was broken.

Employee and another miner were removing pipe flange bolts. Tools used were 3/4 inch air impact gun and a box end wrench. When the miner engaged the air impact gun, the wrench moved pinching the employees finger between the wrench and the pipe flange.

A Contract Driller had finished changing the foot clamp jaws & prepared to change the top chuck jaws with the Driller Helper standing on the off side with EE's hand on the tower bracket on the back of the drill mast. Drill head was lowered and pinched the Helper's fingers b/t the head & lift cylinder mount bracket, avulsing the Right index & middle finger @ 2nd knuckle.

Night shift Maintenance Millwright was prepping a piece of flat bar steel 1/4"x4"x27", using a bench disc sander. The steel was "grabbed" by the sanding disk and struck and punctured the hardhat, contacted the hardhat suspension inside, and continued downward striking the millwright just above the left eyebrow resulting in seven sutures.

Employee was using a drill to bore a hole. Due to torque the drill kicked back striking employee in lip. The employee received a minor laceration which was closed with three sutures by the operator's medical clinic and the employee returned to duty without restrictions.

Employee was performing an overhead weld on a 993K loader bucket that was inverted in the weld shop, after finishing a weld he leaned to the side to inspect the new weld and a small piece of slag popped from the weld and went under his safety glasses contacting his left eye lid.

Operator was sitting down with a jack hammer in his lap with the bit facing the chute. He was having a conversation with the other operator when a large slab of Pb concentrate fell off the wall of the chute and struck the bit of the jack hammer. This made the jack hammer hit him in the left thigh.

Injury was cutting the frog off the dozer blade on 9712 in north # 3 bay. He felt heat on the lower right calf, looked down and found coveralls on fire. He used both gloved hands and put the coverall fire out, the sweat pants he was wearing beneath the coveralls were on fire also. By the time he extingished the sweat pant he sustained a burn on the right calf .

While the Mechanic was removing a 5th wheel plate off a truck with an air impact gun and a universal socket, the socket slipped off the bolt and came off the air gun hitting him in the mouth, chipping a front tooth & cutting his upper lip.
