An outside truck driver came onto this site to get loaded where the driver then parked their truck and was out of view from out scale house. Driver began to work on the back of the truck where the air bag blew out from under the truck and hit the driver in the head. Driver was not our employee, nor was the driver supposed to be working on their truck.

Showing all 4An employee was fatally injured during maintenance work on the hopper. EE was operating a CAT forklift in close proximity to a Volvo excavator. EE exited the vehicle without notifying the other employees working in the area. Other employees stated they heard a "pop" and found the forklift operator unconscious next to the fork of the forklift.
Sub-contracted tow truck was dispatched to tow super sucker truck. Boom of tow truck was being operated by tow truck employee #1 when it struck tow truck employee #2 in the head. Employee #2 was taken by ambulance to local ER for treatment where he later passed away. Most data unknown due to HIPPA and subcontractor status of deceased
WHEN POSITIONING THE DRILL, THE RIGHT REAR LEVELING FOOT SANK INTO A MUD SEAM ALLOWING THE DRILL TO OVERTURN. THE DRILL SLID OVER THE EDGE OF HIGHWALL AND THE ROCK LEDGE CRUSHED THE DRILL OPER ATORS COMPARTMENT CAUSING DEATH OF DRILL OPERATOR.
