Employee was going to strap rock that was stuck in cone, the plant man ran the cone cleanout backwards causing the rock to fracture. Part of the rock struck employee in the face.

EE acted against policy/training by leaning a ladder against running screen box to clean the screens. EE began to descend ladder when it began to slide. EE reached out with left hand to steady ladder, contacting flywheel of screen box, resulting in injury. EE returned to full duty on 10/30/17, retrained on proper way to clean screens, fall protection and lock out/tag out procedure.

Was removing a lock nut on a jaw toggle spring. The bolt securing the tension on the spring broke and trapped his left hand between the air wrench and motor base.

EE HAD JUST WALKED OVER TO THE IMPACTOR FROM CHANGING A SCREEN WHEN CO-WORKER ASKED EE TO PULL THE SAFETY PIN SO HE COULD ROTATE THE BARS. WHEN HE PULLED THE PIN THE SHAFT STARTED TO ROTATE & CO-WORKER WAS STANDING ON IT. EE REACHED UP TO STOP THE ROTATION WHEN HE GOT HIS FINGER CAUGHT BETWEEN THE BAR & FRAME.
