Showing all 23Employee was using overhead crane to place ball mill door on mill. As the door was being lifted employee's left thumb was crushed in lifting block.
Employee was using an air power impact wrench wearing latex gloves. The safety pin began to back out of socket. Employees hand was holding socket. Safety pin caused laceration to palm of hand.
Employee was working with a crew clearing accumulation at the 8x20 primary screen. Employee walked away from primary screen to inspect #3 Drive Motor Belts. While looking up to inspect the belts, a falling rock struck the employee on EE's hard hat / face.
The vent cover of the in cab overhead AC unit (air mover)fell off. The operator attempted to replace the vent cover, as they pushed up on the vent cover to place in back, in the air mover, it slipped causing their fingers to enter the unit. The fan blade struck the operators fingers causing a laceration on the middle and index finger of the right hand.
Employee while grinding lost body position with grinder and lacerated left pointer finger, left and right thigh, and sprained right knee
While using a cutting torch, above ground. the slack hose caused the torch to slip off his left hand and burn the forearm on his right hand.
Alleges, that on Friday while he was welding he felt something hot. Boot is not burnt on the outside, No indication of burn. (Note: employee is diabetic and is not seeking Dr. treatment as of today).
EE was cutting bolts holding up plate liner and it fell on top of left foot causing a fracture.
While working on RM-310, the table was hot. He received a burn to his shin on right leg, it was not severe, but he got it infected.
Contractor mechanic was using a right angle grinder for plant repairs. Grinder recoiled and struck the mechanic on the lip. Result: laceration to the lip, eleven stitches needed.
Reported on 8/19/2008: Employee was cutting overhead bolts and hot slag fell on to both arms causing small burns. This job ended on 8/14/2008.
IP was using a air powered cutoff tool to trim a bolt. IP was wearing safety glasses. Debris passed between the safety glasses and the IP's face and entered the IP's eye. IP reported the injury the following day and was taken to clinic for treatment. IP was put on light duty for 3 days.
While performing welding operations, a piece of hot slag got into his shirt and worked its way down to his waist line. This is a forth attempt to submit, the first attempt was made on 5/26/08, but did not receive the submission verification.
IP was removing a damaged bed lift cylinder from a haul truck. As IP was cutting the polish rod with a torch, the rod gave way. The barrel end of the cylinder struck the IP as he was attempting to get away. The barrel came to rest across the legs of the IP. The incident resulted in a fractured right tibia and left knee.
The employee was working on the secondary plant discharge line removing a bolted cover on a check valve. He was positioning a wrench on a nut when co-worker activated a 3/4" drive impact wrench on the bolt head. This made the wrench spin with the nut and EE's finger was caught in a pinch point between the wrench and the pipe flange.
While operating a cement bagging machine, the cement bag broke, blowing cement up to operator. The cement flew up to the face of the operator and between the safety glasses.
Employee (bystander) was standing away from other employees trying to unjam rock bridge in crusher. The (primary) employees were wearing faceshields. While lowering a wedge down into crusher to move the rock, a piece of rock splintered out and pierced employee (bystander) in cheek.
EE WAS USING A DIE GRINDER WHEN THE WHEEL BROKE APART CAUSING A FOREHEAD LACERATION.
WHILE STRETCHING HIGH TO REMOVE BUILDUP IN MARL BIN, HE FELT A PULL AND PAIN TO THE STOMACH AREA.
WHILE LOWERING A THREE STAGE HYDROLIC CYLINDER FROM ITS NORMAL POSITION TO THE GROUND. HE PLACED HIS FINGERS BETWEEN THE TWO UPPER STAGES, AND THE UNIT DID NOT HAVE PRESSURE WHICH ALLOWED IT T O MOVE AND PINCH THE FINGERS ONTO THE STAGE SLEEVE.
CAN FELL INSIDE AIR CONDITIONER VENT AND HE REACHED IN TOO FAR TO RETRIEVE IT FINGER TOUCHED BLOWER FAN.
WHIEL CUTTING W/TORCH THE PIPE HE WAS CUTTING ROLLED OFF THE TABLE AND LANDED ON OXYGEN & ACETYLENE HOSE CUTTING THEN IN TWO, HE TRIED TO CRIMP THE HOSE USING HIS HANDS, BUT HIS L HAND SLIPPED ONTO THE HOSE.
