While cutting through a retention bracket that held down a compressed spring, with a welding torch, the force of the spring popped up, causing a 1" piece of the retention bracket to ricochet against employee. As a result, employee sustained a laceration and contusion to the left chest area.

While operating overhead Gantry Crane controls with right hand, employee guided and latched hook into the completed catwalk with his left hand. Upon pressing the up button to raise the catwalk, the hook twisted, crushing his left thumb between the hook and the catwalk causing laceration.

While using the impact gun to unscrew 3/4" flat head bolt on a backhoe bucket, employee pushed on the flat head bolt with his left hand to prevent it from spinning. The flat head bolt began to spin and the edges of the bolt sliced into his left index finger causing a 1/4" deep laceration that required 5 sutures.

On 2/17/09, employee was operating a Dozer (equip# 2550) to remove a toe (rock protrusion) that required more time and force than normal to remove. At the end of the day employees back was sore from all the jolting of the equipment. Employee worked a full shift the next day and his back was still sore. He reported the injury to the supervisor after his shift on 2/18/09.

While working in the parking lot behind the lunch room cutting down trees, employee was standing behind the tree that another employee was cutting when the tree slid back and hit employee in his inner right thigh.

EE was welding threaded studs from morning till lunch. After lunch he noticed numbness in his right leg from hip to foot and thought that his leg "fell asleep". He came into work the next day and his right leg still had numbness.

While working with another employee to separate and park the tractor from the trailer using a forklift and chains. Employee began to pull on the chain to position the lift, when the chain got stuck and pulled employee's left shoulder out of socket.

WHILE OPERATING GRADER IN PIT,TRAVELING UP HILL IN REVERSE, A 50TON TRUCK WAS REVERSING DOWN THE SAME RAMP; GRADER & TRUCK COLLIDED. HAD PAINIMMEDIATELY IN HIS LOWER BACK & NECK AREA.

WHILE EMPLOYEE WAS HOOKING UP AIR COUPLING TO IMPACT WRENCH, AIR GOT INTO THE PUNCTURE WOUND ON HIS LEFT HAND NEAR HIS THUMB. THE PUNCTURE WOUNDWAS FROM A PREVIOUS INJURY.
