EE WAS WEARING SAFETY GLASSES TO CUT A PIECE OF METAL WITH A CUTTING TORCH. HOWEVER, HE SHOULD HAVE WORN A FULLL FACE SHIELD

AFTER A WELD PASS WAS COMPLETED, A NEEDLE SCALER WAS USED TO LOOSEN THE WELD SLAG. THEN AN AIR NOZZLE WAS USED TO BLOW AWAY THE SLAG. THE AIR BLAST FROM THE NOZZLE BLEW A PIECE OF SLAG INTO TH E EE S LEFT EYE. EE USED EYE WASH BUT DID NOT REPORT THE INCIDENT UNTIL THE NEXT DAY. EE REPORTS HE WAS WEARING SAFETY GLASSES.
