EMPLOYEE WAS CLEARING A PLUG IN THE PRE-HEATER BUILDING THROUGH A POKE HOLE. HE WAS NOT WEARING THE PROTECTIVE COAT, FACE SHIELD & USING A SHORT AIR LANCE. THESE ITEMS WERE AVAILABLE FOR USE. HOT FEED BLEW OUT THE POKE HOLE & CAUSED SECOND DEGREE BURNS ON ARMS, BACK & FACE.

THE ARTICULATED TRUCK WAS HAULING DIRT WHEN THE FRONT PIN ON THE RIGHT STEERING CYLINDER CAME OUT & THE TRUCK TURNED SHARP TO THE LEFT ROLLING THE BOX OVER & WENT OVER AN 8' EMBANKMENT. THE DR IVER SUSTAINED SOME CUTS ON HIS LEFT ARM AND ELBOW AREA FROM THE GLASS OF THE DRIVER'S DOOR. THECAB DID STAY UPRIGHT.
