In the process of changing hammers, EE was unscrewing it and lost control of the hammer injuring left index finger. (Cut & bruised.)

Dredge pipeline was plugged, EE turned on the booster pump to clear the line. The EE was standing next to the plastic pipeline when it came apart. The pipeline hit the EE slamming him into the I-Beam on the booster framework. The EE had his leg broken in 3 places and his ankle bone was crushed.

Employee was using a pressure hose cleaning around the wash plant. He was washing around the log and was in a tight area. He felt his back strain.

Employee was drilling holes mounting a fender. The drill bit caught and forced the employee's hand through a head light cutting his hand.

Injury was operating a bull dozer and he was hooking a lime bin to the dozer so the bin could be moved. After the bin was hooked up and ready to move he noticed a rock underneath a wheel on the bin and attempted to remove it. As he did this the dozer pulled forward. The wheel went over his arm, shoulder, back, and center of his body.

Employee was loosening a wheel on a press when it suddenly released, causing employee to lose his balance and let go of wheel with right hand. His body swung around to the side on press while he was still holding on with his left hand. MRI was done 10/13/06 and informed on 10/18/06 that surgery was required to repair tear.

EE WAS WORKING IN SHOP DOING GENERAL REPAIR TO INCLUDE CUTTING AND GRINDING. EE IS NOT CERTAIN WHEN THE INJURY OCCURRED. EE WAS WEARING SAFETY GLASSES. EE THOUGHT OBJECT CAME OUT OF EYE BY IR RIGATION. A COUPLE OF DAYS LATER WAS TAKEN TO OPTOMETRIST, UPON EXAM, A PIECE OF STEEL IMBEDDED IN HIS RIGHT EYE, WAS SUPERFICIALLY IMBEDDED, SPUD WAS USED TO REMOVE OBJECT WITHOUT COMLICATION

DRILLING WITH A HAND DRILL WHEN THE DRILL BIT CAUGHT, RESULTING IN THE HANDLE TO SPIN A PINCHEING THE EE HAND BETWEEN THE DRILL AND A TRASMISSION PAN. RESULTING IN A 1.5 CM LACERATION RECEIVIN G 2 SUTURES.

WORKER STRUCK THE DOOR WITH THE HANDLE END OF A LARGE CRESCENT WRENCH. AFTER STRIKING THE DOOR, THE WRENCH TRAVELED DOWN THE CHUTE FOLLOWING THE ANGLE. UPON REACHING THE BELT, THE WRENCH HANDL E WAS DRAWN UNDER THE CHAIN AND PULLED INWARD. PINNED HIS HAND AGAINST THE GUARD OVER TAIL PULLEY, IT PINCHED AND PULLED OFF ONE HALF OF THE INDEX FINGER, ALSO A THUMB LACERATION.
