EMPLOYEE WAS TOLD BY DRILLER TO HOLD ONTO THE BOTTOM SLIPS WHILE ANOTHER EMPLOYEE TRIED TO LINE UP TOP SLIPS. HE HAD TROUBLE, SO DRILLER RAISED COLLAR, FORGETTING ABOUT JAVIER. THIS RESULTED I N PINNING HIS MIDDLE AND RING FINGER CAUSING INVOLVED FINGERS TO BE CUT.

EMPLOYEE WAS RELEASING A SECTION OF PIPE FROMT HE JAWS OF A FUSING MACHINE. THE PIPE WAS UNDER TENSION AND SPRANG UP, KICKING THE HINGED JAW INTO HIS CHIN. EMPLOYEE RECEIVED SEVERAL STITCHES TO CLOSE A SMALL CUT IN HIS CHIN. NO PRESCRIPTION WAS GIVEN.

THE EMPLOYEE WA8 HOLDING SOME PIPE WHICH WAS BEING CUT BY THE WELDER. HE WAS HOLDING THE PIPE BELOW THE CUTTING AREA WHICH EXPOSED HIS HANDS TO THE SLAG FROM TEH ARC WELDER. THE SLAG FELL INSI DE HIS RIGHT HA3D GLOVE AND CONSEQUENTLY BURNED HIS HAND.
