Employee was using a cutting torch to cut scrap metal. The torch caused a small brush fire behind the employee which caught their pant leg on fire, burning through two layers of clothing. Employee sustained a partial thickness second degree burn on the back of left leg just above the ankle.

The employee was cleaning a cement coupling with a drill grinder. The drill bit became dislodged from the drill and lacerated the employee's left index finger.

Grinding hinge door bolts off with an 4.5 grinder when the grinding wheel came apart striking technician in the left side cheek area from lip to upper cheek, EE was wearing safety glass, no face shield. EE received medical treatment from Emergency room and was released same day and put on light duty next 5 days.

Unloading material from a skid. On the last pick EE found a nylon strap still attached. EE reached in with the cordless grinder to cut the strap. When the strap was cut the load shifted and EE's thumb was caught between the load and the diagonal brace on the skid.

Incident occurred while employee was using rotary hammer from man-basket to remove build up on crane way wall and pipes. Employee was hammering build up on pipes when it became lodged in build up. Employee let off the trigger was a few seconds then engaged trigger to try and pull out hammer. While pulling on hammer employee felt pull/ strain in middle/ center of back.

Four employees were stowing the jib on a Link-Belt RTC 8090 crane and were on the platform of the crane. The jib was not adequately stowed correctly; the crane operator telescoped the boom out, and the fly-tip head sheave was pushed towards and bent the front fly storage bracket. This caused the jib to swing out violently, striking two of the four employees.

Four employees were stowing the jib on a Link-Belt RTC 8090 crane and were on the platform of the crane. The jib was not adequately stowed correctly; the crane operator telescoped the boom out, and the fly-tip head sheave was pushed towards and bent the front fly storage bracket. This caused the jib to swing out violently, striking two of the four employees.

The miner was using an angle grinder to grind a weld when the grinder kicked back and flipped the miner's face-shield up and struck the miner's lower lip, causing a laceration.

The employee was grinding on a steel plate. Had all PPE on for the required work, EE stopped grinding, removed EE's PPE and wiped EE's forehead with EE's sleeved arm and the steel speck must have entered EE's right eye at that point.

A miner was using a pneumatic die-grinder to prepare the steel surface on a dump truck for welding when the miner felt something go into eye. The miner flushed eyes several times & finished shift. The next day the miner experienced discomfort in the eye & sought medical attention. A foreign object was removed from eye & miner was released to work with no restrictions.

Employee was guiding excavator bucket to pull bridged rock back into feeder. As the teeth of the bucket grabbed the rock the rock fractured and a piece struck the employee in the chin, resulting in six stitches.

EE reported their right thumb was sore on 5/29 from operating the Caterpillar D8. EE was evaluated on 5/29 with a strained right thumb and put on modified duty. On 5/31 EE was evaluated and released to full duty.

Employee was using a vise grip to secure a piece of plate while they were drilling holes. The drill became bound up in a hole spinning finger against the vise grip, pinching pinky finger against the vise. Employee was given pain meds, antibiotics, and removal of fingernail. Returned to work the next day.

EE reported had a sore back from operating 65 drill on 7/13/16. EE was medically evaluated on 7/13/16 and put on regular duty. EE went back to doctor on 7/25/16 and was prescribed physical therapy and muscle relaxers.

Employee was grinding on a conveyor belt splice. Employee finished EE's shift, and the next day reported pain in EE's right eye. Subsequently the employee was taken to the doctor, and a piece of metal shaving was removed from EE's right eye. Employee was wearing safety glasses, but was not wearing a face shield.

Employee was using a cutting wheel on a grinder to cut 1x1 inch rectangle steel. The EE stated that the wind kicked up blowing dust into his eyes and he lost sight of the cut off wheel causing it to hit and kick off the steel and hitting the employees right leg just above the knee causing a 3 inch laceration. This laceration required 15 stitches to close.

The task being performed was changing wear steel on a screen. The repairman was backing out a bolt with an impact gun when it spun out and cut his finger. The extent of the injury was a laceration to the left index finger requiring three stitches. The employee was wearing gloves when this incident occurred.

EE was cleaning the built up material on the burner pipe for the kiln when he said he felt a sharp pain in his left shoulder. EE did not report the injury until 8/30/13 and refused a medical evaluation. EE requested to be seen by Dr on 9/19/13 and EE was placed on modified duty that day.

EE was attempting to extract a 30 ft pipe from the ground using a backhoe. The EE rigged the pipe with a 30' sling and attempted to pull the irrigation pipe out of the ground, while attempting this the rigged end of the pipe broke free striking the EE in the head when it entered the cab. The laceration required 7 stitches to close.

Employee was disassembling a 30 HP water pump for inspection when he took off his gloves to remove the bolts, he then turned the impeller without gloves and lacerated his right index finger. The finger required 9 stitches.

An operator assisting an EE Agg Delivery Driver was straightening the tailgate of his Highway Dump Truck. EE was holding 4"x4" wood block with his gloved left hand against the tailgate, when the operator applied pressure with the 330 excavator' s bucket, the block rolled, pinching EE's left thumb between the block and the bucket requiring removal of the thumb nail and 3 stitches.

Employee was using a 4" grinder with flapper wheel attached in a kneeling position. Upon completing his task effected employee released the trigger and started to stand up when the back of his right hand came in contact with the still rotating flapper wheel causing a laceration to the back of his hand requiring 4 stitches.

The mobile crane had set the empty spreader bar on the trailer-on its edge. The crane operator was then given the sign to lower the hook, in order to disconnect the top rigging. As the hook was being lowered the spreader fell over and hit the right ankle - soft tissue injury (Note: incident occurred during the de-mobilization of the crane)

Affected employee was operating a dozer during his shift. He complained of back pain but indicated that no particular incident had occurred causing this pain but thought it was from operating the dozer. The employee was taken for medical observation where it was determined that he had a strain to the lower back. He was placed on modified duty.

Employee was unplugging a chute and while he was hitting the chute he had his hand resting on the air ram for the diverter gate and when the plug broke free the diverter gate opened causing the air ram to to pinch his right hand. He factured his middle finger and has a small laceration that required sutures and abrasion.

Employee states that while he was holding onto a piece of conduit, while cutting with his other hand he felt pain in his left shoulder. He did not report this until 05/01/07 and went to the doctor on 06/07/07. Employee started losing time due to surgery on 10/16/07.

While the employee was removing a wear sleeve from the crankshaft of a 773 cat truck, he was using a torch to heat the sleeve, the flame caused the vapors from the cleaning solution to flash causing a burn to his nose and cheeks.

Employee was using the overhead crane to move a 6"x7' long steel shaft. The shaft had a groove cut in each end for a keeper ring and also had two steel bushing on the shaft. When he lifted the shaft, a bushing on one end slid off of the shaft striking his finger cutting off 1/2'' of the tip of left middle finger.

Operator was operating excavator with door open. He was sorting large boulders when he dropped a boulder on the pile and a chip flew in the cab through the open door striking him in the left eye. Operator was not wearing safety glasses. He was examined by an optometrist, given prescription eye drops and sent back to full duty.
