A mobile jib crane was being used to lift a piece of grating. When the grating did not move the crane was readjusted while still attached and under pressure. The grating cross bar failed and caused the chain assembly and crane arm to strike the EE in the face. EE was treated by MD and taken off work.

EE was welding a plate from the underside of a bucket and when he began to pry the plate off, it broke free, hit the ground and flipped onto the ee's R foot. He was evaluated and given restrictions to return to work. He returned to full duty on 12-31-15.

Mechanic installed a distribution hose on portable steam cleaner. When he started the steam cleaner hose came off, noise, caused mechanic to jump back twisting knee. Employee was evaluated and provided restrictions the department accommodated as normal work duties. 11/5/15 determined duties performed are not part of normal job duties and thus to be counted as Restricted Duty Days.

EE alleges back pain from leaning forward in an awkward position to weld. On 9/19 it was made known that he alleged a missed shift on 9/15 as related to his work injury. He was not disabled off of work by a health care provider. EE had self-treated with a chiropractor and on 9/24 the workers comp carrier determined to accept chiropractic tx with his comp claim.

Employee alleges right eye pain. No specific event stated; employee reports he'd been welding & grinding at work frequently. He was evaluated by a MD and a foreign object was removed. He was given Rx eye drops and narcotic pain medication. He missed his 8/15 shift due to taking pain medications. He was scheduled off 8/16-17 & returned to work full duty on 8/18.

Employee was backing a dozer up and reported that he hit a chunk of rock, causing him to be jostled in the dozer and experience back pain. He was evaluated by a physician and taken off of work.

The employee was using a grinder to perform work when the grinder "jumped" and caught his left index finger, pulling it between the guard and cutoff wheel. He was evaluated by a physician and four sutures were used to close the wound. He was released back to work with restrictions to keep the wound clean and dry.

Employee was cutting a burn ring when a piece of hot slag entered into his boot, causing small burns to his left foot. Employee treated with his personal doctor on 9/26. He was taken off of work and missed his scheduled shifts on 9/25-9/27. Employer was made aware on 9/30 that the missed time was related to his to his reported 9/5 work injury.

While using an air chipper to chip rust off of a beam, employee felt a foreign object enter his left eye. He was evaluated by a physician. The foreign object was removed using a cotton swab and the employee was given a prescription for antibiotic eye drops.

On 1/23/13 a chunk of material rolled off of a mining face and made contact with the shovel, causing the shovel operator to be jostled. He was evaluated by a physician on 1/23 and released back to full duty work. On 10/10/13 he was prescribed physical therapy. He completed a "series" on 10/21/13.

While using a pneumatic grinder with wire wheel, the grinder kicked back and struck the employee in the right knee, causing a laceration. The employee was evaluated by a physician on 12/7. Dermabond medical glue was used as an alternative to sutures to assist with wound closure. The employee was released back to work full duty for his next scheduled shift on 12/10/12.

On 9-7-12, the employee felt a pop in his left shoulder while lifting an impact wrench (8 lbs) to his co-worker. He was evaluated by a Physician, given an immobilizing sling and returned to work with restrictions.

EE experienced pain in his lower back while working inside the line 7 feed chute. He was evaluated on 2-17-12 and taken off of work. He missed one scheduled shift. This may potentially be related to a previous incident in January 2012.

Employee was tightening down bearing caps with an air impact, dirt blew up from the table he was working on and got into his eye. He was evaluated by an Opthalmologist, the FB was removed and he was given prescription eye medications.

PMCI employee & a Mill person were standing inside a fan housing, pushing up on the fan to rotate the fan. The fan started to move & injured's head got between the fan and the side of the housing. The other 2 people immediately stopped the fan rotation and pushed down freeing his head. His ear was cut and required stitches.

Employee was welding on a corner bit for a grader blade. The edge was on the work bench standing up. The edge blade began to fall and employee tried to catch it jamming his finger against the edge. He suffered a fracture of the finger.

Employee was working at Unit 2 Kiln cooler dump point. While they were observing pellets in the cooler from the area of the access doors, a blast of hot dust and steam from inside the cooler knocked open the steel doors and pushed the employee backwards. The hot dust contacted him causing first and second degree burns to his head and neck area resulting in loss time.

Employee was cutting directly overhead when a piece of slag landed on his glove. While brushing the slag off his glove he inadvertently swung the torch across his right leg, resulting in a burn to his leg. He was evaluated by a physician and taken off of work for 2 days: 2/28/06 and 3/1/06.

While guiding a steel pipe using an overhead crane, the pipe shifted and employee's hand got pinched between the pipe and the steel rack on the truck bed. He received 4 stitches in his left hand.

EE WAS USING A CUT OFF WHEEL ON A AIR GRINDER TOCUT OFF THE RAILING ON A PIECE OF EQUIPMENT. THEWHEEL ON THE GRINDER BLEW APART STRIKING HIM ON THE LEFT SIDE OF NECK RESULTING IN A LACERATION.

EE AND HIS PARTNER WERE WORKING ON UNIT 4 ROOF MONITORS. THEY HAD TO REMOVE SOME SHEETING TO GET AT THE LOUVERS. WHEN USING AN IMPACT TO REMOVE THE SHEETING SCREWS, ALL THE BOTTOM SCREWS WERE OUT AND WHEN HE STARTED TO REMOVE THE CENTER SCREWS THE SHEETING BROKE OFF STRIKING EMPLOYEE'S LEGS RESULTING IN A CONTUSION TO HIS RIGHT LEG.
