I,NAME WAS STEA4 CLEANING HIS LOWER. HE WAS UP ON THE LOADER AND JUMPED FROM ONE PART TO ANOTHER WITH THE NOZZLE IN HIS HAND AND STILL DISCHARGING STEAM. THE END OF THE NOZZLE STUCK IN HIS SHO E AND SPRAYED S7EAM ON HIS FOOT CAUSING 2 BURN.

I.NAME WAS CUTTING METAL ON A PIECE OF HEAVY EQUIPMENT WITH A CUTTING TORCH. HE HAD GLASSES ON, BUT THEY FOGGED UP AND HE TOOK THEM OFF. HE GOT A SMALL PIECE OF RUSTY METAL IN HIS LEFT EYE.
