Employee was grinding on steel and felt something was in employee's right eye. There was debris falling from above. Employee did not report this until the next day.

While an employee was loosening house bolt on S33 with an impact, some debris got into eye. EE tried to flush debris from eye on site. EE was brought to the hospital where a foreign body was remove from eye and employee returned to work. Safety glasses were being worn at the time of the incident. Prescription eye medication ordered.

Alleged: EE stated on next shift that EE got something in eye when needle scaling the previous shift. States self flushed and was wearing safety glasses and face shield at time of event. On 4-20-17, EE was evaluated in the ED and given rx medication for eye.

EE was using a hand held grinder with a cut off wheel to make modifications to a conveyer belt guard when ee lost control of the grinder striking right leg just above the knee causing a laceration. Taken to local emergency room. 6 stitches required to close laceration. Returned to work with restrictions.

Tilden Plant Repairman was torqueing bolts on #2 Dryer with a high torque hydraulic wrench. Employee placed hand on wrench and asked partner to apply hydraulic pressure. At this time his finger became caught in between the wrench and bull gear flange, resulting in 13 stitches. Employee is off work on administrative leave post drug and alcohol testing.

Alleged aggravation of a pre-existing injury from 1-15-07 back strain from slipping on deck of drill while shoveling snow and 12-16-09 neck strain from being jostled in loader. EE continues to state pain in lower back, neck while operating mobile equipment. The employee was evaluated by a Physician & physical therapy was ordered. The EE has completed a series of visits.

Employee states he had been welding and grinding in previous days with adequate PPE worn and no eye irritation or event reported during the time of welding or grinding. Days later employee was seen and treated for a foreign body in eye.

Employee alleges that a foreign object entered in his right eye while grinding. He was evaluated by a physician on 10/9/14 and a piece of metal was removed. He was issued antibiotic eye drops and taken off of work for one shift (10/10). He returned to work full duty on 10/11.

On 12/29 employee alleged that during his previous shift (12/28) he was jostled while shifting a dozer from forward to reverse, resulting in back pain. The employee was evaluated by a doctor on 12/29 and issued Rx muscle relaxants & released back with restrictions. He was unable to report for 2 shifts due to the side effects of the muscle relaxant. He returned full duty on 1/1/14.

The employee was holding a pulp lifter in place while another employee was cutting the last bolt. The pulp lifter slipped and struck the employee in the forearm resulting in a laceration. The ee was evaluated by a Physician and sutures closed the wound. He returned to work.

Employee was cutting floor grating. The cutting saw bound up and when it freed up a piece of the steel gave away and pinched off the tip of his finger. He was evaluated by a physician and was taken off of work.

Employee was drilling hole when bit got stuck. Torque on drill caused a clockwise quick motion of drill, catching his left pinky finger, resulting in a fracture and laceration of finger.

Employee was using a sawzall to cut conduit, the saw blade was pinched by the conduit. The saw then jumped causing the employee's hand to slip into the collect resulting in a fracture and sutures to his finger.

The medical was reviewed and it was made known on 1-17-11 that it was work related. Physical Therapy was ordered and a series was completed. The employee was driving his dozer, backed into a feeder and was jostled, resutling in an injury to his shoulder.

Employee alleges that he got a piece of metal in his left eye from air chipping on 12/6/09. On 12/7/09 he reported this and was evaluated by a physician. An imbedded foreign body had to extracted from his eye.

EE was removing drive mounting bolts.When rmoving the 7th of the 8 bolts the remaining bolts sheared off.The stored energy in the drive caused it to rotate rapidly.The belt guard on the drive struck employee in the left shoulder-throwing him forcefully into a handrail and structural beam.He was evaluated by a DR and taken off of work.

He was uncertain to actually when it went in to his eye--he felt irritation in his eye on 2/15/08. While attempting to reattach a scale sensor unit the set screws on the coupler caught employee's glove and started to twist his hand around the shaft. The glove was torn apart allowing his hand to be released. Employee suffered a strain to the wrist and was returned to work with a splint.

The impact wrench used to re-torque the spider bolts in the crusher fell off the end of the monorail that it was suspended from. The wrench struck the employee on the foot causing a fracture.

EE WAS INSTALLING ROLLERS USING A COFFING HOIST TO RAISE THE ROLLER FRAME TO A VERTICAL POSITION ON THE CONVEYOR BED. HIS LEFT HAND WAS BRACED ON THE NEXT ROLLER FRAME DOWN WHILE HIS RIGHT HAN D WORKED THE COFFING HOIST. AS THE FRAME WAS BEING ROTATED INTO PLACE IT SUDDENLY LET LOOSE AND SLID DOWN AGAINST THE NEXT FRAME, CRUSHING HIS FINGER. THE WOUND REQUIRED A STITCH.

EE PUT HIS FINGER OVER A HOLE IN THE HYDRAULIC HOSE ON A LOADER, WHEN HE WENT TO RAISE THE BACKHOE BOOM, THE PRESSURE IN THE HOSE SHOT UP TO 2500 PSI WHICH INJECTED HYDRAULIC OIL INTO HIS FING ER. EE WAS EVALUATED IN THE E.R. HAD SURGERY ON HIS FINGER AND WAS TAKEN OFF OF WORK.
