Employee was helping a welder by holding the metal pieces while the welder tacked the pieces together. During the welding process, employee looked at the arch flash, which caused cornea flash burns to both eyes. Requested medical visit on 1/21/2021 and was prescribed medication for eyes.

Employee was using a cutting torch to cut off a shaft. He needed to switch the torch from his left hand to his right hand. As he passed the torch from his left to his right hand, the flame shot under his glove and burned the top of his right hand. A 2nd degree burn developed around the affected area.

Employee was using a blower to blow dust off his body. As he moved the hose over his chest and up toward his face the blast of air blew his safety glasses off his face and they fell to the ground. Employee believes dust got blown into his right eye at that moment. Around noon, his eye began to hurt and was swollen and bloodshot. Treated for corneal abrasion.

Employee was a operating a Backhoe. He scooped up a load of material into the backhoe bucket and as he began to raise the bucket, a gust of wind blew material into his face. Beads of sweat ran down his forehead, mixed with the material and ran behind his safety glasses and into his eye. His eye became irritated.

The employee was working on the south bagger chair cylinder. While adjusting the air flow, he bumped the limit switch for the north side and lime blew into his right eye.

The employee requested medical treatment for pain in both wrists. The employee says that he experienced pain while using a grease gun to lubricate machinery. Squeezing the grease gun causes this pain. The employee does not recall any event that might have triggered this pain.

The employee was working inside tunnel #3 on the #4 feeder. He was using an air hammer to loosen material in the feed chute. He was standing on a step. He stepped back off the step and lost his balance due to the weight of the air hammer. The air hammer hit him in his the abdomen and groin area.

The EE was washing down a haul unit with the high pressure washer. He was laying on the creeper. He was wearing his safety glasses & face shield. He felt something scratch in his eye. He used eyewash solution then the eyewash station. He requested medical attention shortly after because his eye was still hurting.

Employee was cutting a metal plate with an iron worker. Employee was preparing clamp to secure the metal for cutting, when his foot accidentally activated the clamp, pinching employee's left middle finger between the clamp and the metal plate. This resulted in a laceration and bone tip fracture of the affected finger.

The employee climbed up the ladder to cut a door to attach the end of the bucket chain to the crane ball. He was standing on the ladder waiting for the crane ball to connect the bucket chain. As the crane ball lowered in the elevator, it knocked lime dust loose from the wall. A gust of wind blew the dust under and over his face shield and safety glasses.

EMPLOYEE WAS WORKING AT BAGGING MACHINE, REMOVING BAG. DUST ON THE BAG GOT INTO HIS TYVEK SUIT AT THE RIGHT KNEE CAUSING THE BURN. EMPLOYEE WAS WEARING JEANS WITH HOLES AT THE KNEES AND WAS UNAWARE THAT THE TYVEK SUIT WAS TORN ON THE RIGHT KNEE.

EE WAS CLEANING THE INSIDE OF THE HYDRATE MILL DUST COLLECTOR, WITHOUT LOCKING OUT THE EQUIPMENT. HE CAUGHT THE TIP OF HIS MIDDLE FINGER ON HIS RIGHT HAND IN THE AIRLOCK, CAUSING INJURY TO THE FINGER. THE TIP OF HIS FINGER WAS AMPUTATED.

EMPLOYEE CLAIMS THAT HE GOT 2ND DEGREE BURNS WHILE HE WAS BAGGING HYDRATED LIME INTO 50 LB BAGS. HE FURTHER CLAIMED THAT WHILE WORKING IN THE BAGGER, HE WAS WEARING LATEX GLOVES AND A NOVEX SU IT TO PROTECT HIM AGAINST LIME EXPOSURE. THE SLEEVE OF THE SUIT WAS TAPED TO THE GLOVES. THE EMPLOYEE WAS SEEN AT PRO-MED 183 CLINIC AND EXCUSED FROM DUTY UNTIL FURTHER NOTICE.
