Employee was spinning anvils in a Vertical Shaft Impact Crusher. The lid of the VSI came down upon his left hand. Employee's left hand was caught between the lid and frame of the VSI. The ring finger of his left hand was seriously injured.

Employee was using an impact wrench to loosen the bolts on the cutting edge of a loader bucket. He reached under the edge of the loader bucket to locate the bolt head. He then engaged the impact wrench. The bolt head on the underside of the loader bucket spun, caught the employee's glove, and lacerated his left middle finger.
