While setting a crusher liner in the jaw crusher, the laborer got in the crusher. The liner slipped, wedging his foot between the liner and a 2x4. He spained his ankle severly.

EE WAS WORKING AS A MECHANIC ASSISTANT AROUND WELDING ACTIVITIES, DURING HIS WORK SHIFT ON 5/10/04. LATER THAT NIGHT HE SUFFERED FROM IRRITATED EYES, PRESUMABLY CAUSED BY WELDING FLASHES.
