HE WAS STRIPPING MATERIAL FROM HIGHWALL AND IT HAD CONTAINED A POCKET THAT WAS NOT VISIBLE. THE MATERIAL SLID OFF AND UPSET THE TRACK-HOE AND SHOVED IT ABOUT 100 FT. VICTIM WAS WEARING HIS SEAT BELT AND WAS TRAPPED FOR ABOUT 2 HOURS AND 25 MINUTES.

EMPLOYEE & CO-WORKER WERE WORKING OUT OF THE MANLIFT BASKET. WHILE EMPLOYEE HELD A BEAM THE CO-WORKER WELDED IT IN PLACE. A SPARK FLEW FROM THE WELDING INTO EMPLOYEE'S RIGHT EYE. HE WAS WEARIN G PRESCRIPTION SAFETY GLASSES. HE KEPT RUBBING RIGHT EYE, CAUSING MORE DAMAGE TO THE EYE.
