Injured was poking the built-up coal out of the chute to the transfer screw using a 1/4" piece of round stock steel. The poker made contact with the screw conveyor below, causing it to pinch their finger and cause a laceration on the ring finger of their left hand.

Showing all 30A contract laborer was positioned on the downhill side of a brick rig used to install kiln brick when they sustained an injury. They had placed their right hand on the brick rig's platform and when the rig was moved a brick fell striking their hand. This resulting in 3 broken fingers.
Grinding on sealing of the kiln, the grinder kicked back, hitting the bottom of EE's welding helmet, knocking it up, allowing the grinder wheel to make contact with EE's face. EE was taken to the hospital where EE received stitches to EE's upper and lower lips. The grinder was equipped with a guard.
On Monday 11/13/2023 management was made aware that an employee had been put on restricted duties. This employee had been previously released with no restrictions back to full duty on 1/10/2023. No new incident was reported to management to support this claim. Employee stated on 11/13/ 2023 scaling causes pain in their back.
An employee was observing the shooting of an ash ring in # 1 kiln when EE felt a piece of material enter left eye. After attempts to wash the eye on site EE was taken to taken to a physician for treatment. EE states EE was wearing prescription safety glasses with side shields and a face shield.
Injured was starting up 6 feeder in 4 baghouse. The feeder started up and the drive started jumping flinging grease which landed on hard hat and glasses. Some of the grease entered left eye.
Employee was repairing a hole in the screen room tin. When the drill bit came through the tin and into employee's ring finger. Initially this was not reportable, but issues arose with the finger causing employee to have surgery to repair a tendon.
Injured was drilling holes to fasten down skirt rubber on a conveyor. When exiting the conveyor hood cover, injured felt pain in left shoulder. Injury became reportable on 09/20/2019 when injured was given work restrictions and referred to an orthopedic specialist.
While engaging the pony motor on #1 Kiln it came out of gear and the clutch handle struck EE's right hand bruising the thumb. 1 day of restrictive duty.
Injured was using an electric jackhammer to remove hard pan material from S-106 feeder in the hical tunnel. While using the jackhammer, the injured felt pain in shoulder. The injury became reportable on 6/8 when the doctor ordered physical therapy.
Injured was using an air chisel to remove ash ring from #2 kiln when a piece of ash fell and struck shoulder/arm. Injured did not request medical treatment until 2/16/2017.
Employee suffered a laceration to right thumb while replacing buckets on a hydrated lime elevator. They pinched their finger between the bucket and chain as they tightened the nut with a pneumatic driver.
The employee was cutting a metal plate with a hand grinder equipped with a cutting wheel to get it to fit. There was some pressure on the plate that caused the grinder to kick back and when it did, the rotating cutting wheel hit ee left forearm and cut it.

Employee was using hand grinder to grind down weld. After shutting off and preparing to set the grinder on the bench, the wheel became tangled in the string of his hooded sweatshirt, which then wrapped up and contacted his jaw causing a laceration.
Employee 1 was assisting employee #2 welding in the shop and received flash burn to eyes.
EE was welding the flange on the top hatch of the kiln 4 baghouse and a piece of hot slag fell onto the tongue of his boot (nylon), burned through, and caused a 3rd degree burn the size of a dime on his foot.
Employee was pressure washing I-beams in an 80 ft boom lift. His hand became wedged between lift basket and steel structure.
Employee suffered burn to right eye while inspecting hydrate product
Employee was underground in the mine at Carmeuse. He was laying on his back underneath the undercarriage of the 320 using a four inch grinder to grind on the undercarriage. Once he was done grinding, he sat up and the dust on the face shield(inside it)fell down and got under his safety glasses and a piece of debris entered his right eye.
Employee was grinding a handrail with a hand-held grinder to prepare for painting. The grinder jerked to the side when he contacted the metal and slammed his hand into the vertical part of the handrail.
Injured was using hand-held drill to drill a hole for a bracket. The drill bit caught in the bracket causing the drill to twist. The twisting of the drill strained his left index finger. Injured did not request medical attention on the date of the incident. Injured later requested medical attention and was given a steroid injection on 3/8/2011.
Employee's right thumb struck the edge of an 8 inch grinding disk while in motion. The employee released the trigger and set the grinder upside down and reached passed the grinding wheel and struck his right thumb on the wheel. Laceration to right thumb requirded 8 stiches and modified work duty and follow up medical care over the next two weeks.
Employee was tramming bench drill to top of bench area, when the drill started sliding on an incline. The drill struck a rib, causing the operator to strike his head inside the cab, which caused a scalp laceration requiring staples.
Employee was working on a screw conveyor. The screw connection required an awkward position for the employee to work in with his arm reaching around the bottom of the screw. While using an impact wrench his arm was twisted because of the awkward position and pulled the pectoralis muscle.
Employee was using grinder to prepare feeder pan for welding repairs when the grinder kicked out striking employee on right leg.
Employee pinched finger in between vertical pipe canopy structure and roof bolter man basket, while the basket was moving into next position. Employee required 4 stitches and was given an antibotic.
EE states that she strained her neck while monitoring the crusher hammer in operation. She had her head turned to right and down some while sitting and operating crusher/crusher hammer.
Employee was installing roof bolt in back located R/N 4 North panel. While attempting to install bolt in drill hole, the bolt got in a bind due to improper machine positioning and slipped out of spinner head causing the bolt to spring towards operator striking his forehead.
