Employee states that he and his bolt partner were backing the roof bolter out of the #5 entry. As he pulled up the cable slack back, the bolter turned and began to roll up the cable to the spool. The cable had a loop in it and as it tightened up, his foot was in the loop. This caused a fracture to his (L) ankle.

Showing all 28Employee states that he was installing a new bolt next to a bent bolt. His hand got caught between the pizza pan and plate of the new bolt and the bent one.
Employee states that while adjusting the leveling chain on the pot he had placed his (R) hand on the boom. His (R) middle finger got caught between short arm and pot arm. This caused a laceration to his (R) middle finger. Employee was wearing leather gloves at the time of the incident.
Employee was bolting #2 right, as he was placing a bolt in the hole he felt pain in his right shoulder.

Employee states he was bolting the #30 wall entry, a bolt was not fully up. He attempted to bend the bolt over by using the drill pot on the roof bolter. As he was bending the bolt, his right index finger was caught between the moving parts on the canopy.
Employee had put up outside bolt & was swinging in to put wide bolt up. Stab jack struck bolt lying on ground and spun it around and the pizza pan struck his nose, cutting it.
Employee was pushing bolt up in hole when the plate cocked sideways on the sissors. He reached up to straighten plate and hit the up lever before removing his hand pinching his thumb between plate and bolt.
Running 16 motor grader, took a hard bounce which resulted in air ride set bottom olut causing back pain. Note: Employee came back to work the next shift and worked til 3/5/09, left job & went to doctor.
Employee was tramming the miner out of #2 entry; he stated that he reached down to put a rope on the miner, looked up and the boom was coming at him.
EE was in W28 face installing roof bolt, after tightening bolt, let pot down on his right knee.

A PIECE OF DRAW ROCK FELL FROM TOP HITTING ROOF BOLTER OPERATOR IN BACK
EE WAS INSTALLING 8-FT-LONG CABLE BOLTS IN #8 ENTRY INTERSECTION. WHILE PUSHING THE ROOF BOLT UP, IT GOT STUCK. EE TRIED TO PULL THE BOLT BACK OUT OF THE HOLE. THE BOLT CAME OUT OF THE DRILL POT ON THE ROOF BOLTER, AND THE ROOF BOLT PLATE STRUCK THE EE ON THE NOSE AND IN THE FACE.
EE WAS ROOF BOLTING IN XR XCUT WHEN A PIECE OF LOOSE ROCK FELL ACROSS T BAR ON ROOF BOLTER APP. 2FT WIDE 1 FT LONG AND 2 IN. THICK. ROCK BROKE THEN HIT THE PERSONAL CANOPY OVER THE OPERATOR TURNING UNDER CANOPY STRIKING OPERATOR KNOCKING HIM DOWN.
OPERATOR WAS PULLING DRILL STEEL OUT OF ROOF WHEN HE LET THE DRILLPOT DOWN ON HIS RIGHT FOOT.
