EMPLOYEE WAS INSTALLING ROOF BOLTS WHEN A PIECE OF METAL CAME OFF THE PLATE ON PIZZA PAN & STRUCK HIM IN HIS LEFT EYE. THE METAL WENT OVER TOP OF SAFETY GLASSES.

SUBJECT WAS EXPLAINING TO AN EMPLOYEE HOW TO CHANGE THE SPEED ON THE DRILL PRESS WHILE THE PRESS WAS RUNNING. WHEN HE POINTED HIS FINGER, IT WAS TOO CLOSE TO THE DRILL PRESS AND HIS FINGER GOT PULLED INTO THE PRESS.
