While installing a 6' bolt and holding the bolt as the pot was raising, the bolt bent, pinching the right thumb at the base of the nail. The pinch caused a laceration of the nailbed and fractures to the finger. Employee was treated and released to restricted duty, no use of right hand.

While roof bolting drilled the first four feet and grabbed a four foot extension. While they were lowering the drill pot they had the extension in their right hand when it caught beneath the pot and on top of their right foot. The steel poked through their boot and into their fourth and fifth toes. X-rays revealed a fracture to each of the toes.

While swinging out the boom on his roof bolter had his hand on the canopy and pinched his left little finger against the coal rib causing a partial amputation. Transported to Memorial Medical Center Springfield where the finger was re-attached.

While escorting visitors and observing the roof bolting process was struck on the back of the left hand by a 6 X 16 roof plate. The impact caused a puncture lacerating the extensor tendon of the left middle finger. The laceration was surgically repaired and he returned to work his next scheduled shift.

While grinding and welding beneath a ramcar tailgate and lying on his back he raised his welding hood and foreign material fell into his left eye. He finished his shift and worked through the next day before continued irritation prompted him to seek medical attention. A piece of material was removed from the eye; he was prescribed medication and then released to unrestricted duty.

While roof bolting a drill steel fell against the side of his neck. The steel was hot from use and caused a burn extending behind his right ear. He continued working and finished his shift before seeking medical attention, the wound was cleaned and he was issued a full release. The application of prescription medication made this a reportable injury.

While roof bolting and adding a drill extension the extension bowed, sprung free and struck his right hand near the base of the index finger. X-rays revealed a fracture and he was placed on a 'no use of right hand' restriction.

While roof bolting and installing an 8' bolt pinched his left index finger between the roof and the plate causing a laceration to the fingertip. Transported to Springfield Memorial ER where X-rays revealed a fracture to the fingertip and the wound was closed with sutures. He was released to unrestricted duty and reported to work on his next scheduled shift.

While operating a continuous miner and cutting the slab side of a straight cut a rock slid off the miner and struck his left foot leaving a laceration. Transported by ambulance to Memorial Medical ER Springfield where X-rays also revealed a fracture to the foot.

While bolting, allowed his left hand to rest on the drill pot where it was struck by a drill steel that had been left in the roof, causing a crushing injury to the fingertip and nail. X-rays revealed a fracture to the fingertip and treatment included removing the nail and suturing to close the wound.

While operating a grinder felt right eye irritation. Followed up next day with a visit to his personal physician, the eye was irrigated and a piece of dark material was removed. The irritation remained and a subsequent visit removed an additional piece of imbedded material. Released to unrestricted duty and continued to report to work for all his scheduled shifts.

While roof bolting he dropped the drill pot and a steel extension fell, bouncing off the pot and striking him on the bridge of the nose leaving a small laceration that required sutures to close. Treated and released to unrestricted duty.

While roof bolting and drilling a hole he lowered the drill pot to remove steel and the chuck adapter came out of the pot. While re-inserting adapter the steel fell from the roof striking the left fingertip resulting in a laceration and fracture. Transported to St. John's Springfield for sutures and released to restricted duty.

While spinning a bolt during installation, a small piece of shale fell and was struck by the spinning plate, causing it to strike the operator on the right side of the face leaving a laceration. Transported by ambulance where the wound was cleaned and sutured. Advised not to return to work until his risk of infection is reduced, minimum 1 to 2 weeks.

While placing covers back onto the right side of #3CM and holding the cover a co-worker attempted to drive it into position catching his left middle finger between the cover and frame causing a fracture and laceration that required sutures to close. Transported by private vehicle to Memorial ER, treated and released, returning to work on his next scheduled shift.

While re-supporting a new beltline intersection & swinging the bolter boom the canopy caught a hanging miner cable & swung the canopy striking him in the back of the head causing a sore neck & shooting pain down the left arm. As he was catching himself he also strained his left shoulder. He did not seek medical treatment until 2/25/2009 placing him on a work restriction.

While truss bolting and removing a steel from the rib the employee's hand tangled with a wad of nylon string adjacent to the hole twisting his hand around the steel. The twisting action tore the skin from the tip of his ring and little fingers. Although no sutures were required the use of his right hand was restricted while healing.

While installing a 5 foot fully grouted roof bolt the operator's glove became pinched between the roof bolt and the plate. As the operator rotated the bolt, his glove was twisted around the bolt, tearing the glove off and injuring his right hand.

Employee was adding extension to drill steel; as he pushed into hole steel jammed and bent, spinning around and struck his right hand. He worked rest of shift. Next morning thumb was swollen. He went to doctor where x-ray revealed a fracture to right thumb. He has been released to return to work.

EE WAS LOWERING DRILL POT ON ROOF BOLTER WHEN HYDRAULIC HOSE CAUGHT A DRILL STEEL THAT WAS OVERHANGING IN TRAY CAUSING STEEL TO FLIP UP AND STRIKE HIM ON RIGHT SIDE OF HEAD ABOVE EAR (ABRASION, POSSIBLE CONCUSSION). DOCTOR RELEASED TO RETURN TO WORK BUT EE CONTINUED TO FEEL NAUSEOUS & FOLLOWING DAY DOCTOR ADVISED NO WORK UNTIL 6/8/05.

Ee was bolting when drill steel bent as he was trying to start hole. He then hit wrong lever and steel rotated hitting right hand first aid was rendered and he worked rest of shift and the following day. Swelling persisted and he went to dr on 12/15/04. Diagnosed with fracture to first metercarpal. Released to restricted duty on 12/15/04.
