Employee was sitting in a 375 Komatsu dozer at the beginning of shift. The other equipment was parked in line behind him. A D11 dozer was parked directly behind the fill dozer and in the process of getting out of line and to the work area, his blade hit the ripper shank on the 375 dozer. At 7:49 pm the employee notified the foreman that his neck was hurting and he was taken to ER.

The D11 dozer track jumped segment sprockets and jarred operator's back. Foreman made note of complaint by employee, but accident was not reported to company until 08/14/2009. After investigation it was determined to be a reportable on 08/21/2009.

While replacing cutting edges on a D11R dozer, employee was using an air hammer to remove the dirt and other material from bolt heads. A foreign object was blown behind his safety glasses and into his eye. He went home and used some eye wash Friday and Saturday night. The pain increased on Sunday and he went to ER.

Adding new drill steel to one of the company's drills. The new steel was put in place by using the boom on the mechanic's truck with a strap around the steel. After several attempts of lining up the steel and jogging the head to thread the steel, EE got his hand caught in the strap. When the head was jogged it wrapped his arm around the steel.

Employee was assembling a new 993 wheel loader. Employee was removing bolts that secured rollover protective structure to shipping pallet. When employee cut the first bolt the ROPS shifted breaking the other bolts and fell on employee's foot.

Employee stated that while running the Kamatsu 375 Co. # 064 dozer on the fill he backed over a rock and jarred his back which he had hurt on 2-02-08 about the same way but had not reported it. He was transported to the ER. He was X-ray and told everything looked fine but on a follow-up told that there might be something wrong with lower back.

Employee was building a road above the Taylor Seam in recently shot material. As he was backing up he ran into a rock that had rolled in his tracks. This caused the dozer to come to a quick stop, that jerked and jarred his back. He finished the shift and on his way home he came back to tell foreman of incident. He worked the next shift then has been off since 7/14/2007.

Employee was backing dozer over fill material when he backed up and over a rock. When the dozer dropped off of the rock it hit hard causing pain to lower back and tailbone. Employee finished shift and continued to work until 11/06/2004. After a trip to an emergency room he was told his tailbone was fractured. At present time employee is still working.
