Showing all 33Employee moved the RBM into the 3 left crosscut and dismounted the operator's deck. As he was exiting the deck of the RBM, the continuous miner trammed into the flypads directly behind the roof bolter, striking the employee and knocking him to the mine floor. This resulted in multiple sprains/strains of the upper body.
Employee stated that he was pulling back on a roof bolt to line it up with the drill pot. As he applied pressure on the bolt he felt something pull in his right shoulder. This resulted in a right shoulder strain.
Employee was mounting the walk-thru roof bolt machine and as he stepped up onto the back of the bolter he struck his head on the thread end of a torque tension bolt that was sticking down from the top. This caused him to strain his neck.
Employee was bolting on the #3 Section in the #6 Entry 130' inby spad # 12262. The bolt crew had stacked plates and pans on the base of the t-bar where the t-bar meets the frame of the RBM. When the boom of the machine swung, the plates and pans fell off and struck the employee on the side of the rt. knee. Employee was diagnosed with a strain and began missing work on 3/8/13.
This report is being filed under protest and solely to abate citation 9005621. According to MSHA, employee incurred a reportable injury when his welding hood was too light and his eyes were exposed to welding arc. Brody denies that it breached and duty to report an occupational injury and reserves the right to contest the issuance of the citation.
The employee stated he caught his right index finger between a ratchet and miner frame causing an open wound that needed sutures. He was tightening ripper motor mount bolts.

EE pulled wrong lever on bolter causing mast jack to smash his right foot under the drill pot.
The employee stated he injured his right middle finger allowing to rest on the drill pot while installing a bolt and the finger was caught between the drill pot and bolt. He is missing work.

The employee stated he reached to activate the joy stick control on the operator side of the machine to push up and spin the bolt and strained his right shoulder near the scapula. he is missing work.
Running roof bolter & drill steel struck top of hand & bruised hand & swelled up. Didn't ride ambulance.
The employee, #2-section, was cutting on shields for cont. miner, hot slag (Foreign body) went into eye. The hot metal slid off down onto pool of water on top of miner frame and splashed underneath safety glasses into right eye. He had lowered his cutting hood.
Upon an investigation, the employee was operating RBM drill head with drill steel in rib. He attempted to remove the drill/pin steel and got his glove caught, causing a compound Fx to his left ring finger.
