After completing some grinding activities the employee states they felt some dust or material fall on their face when they raised up the face shield. Employee also wiped their forehead off to clear the dust. Eye wash was used to flush the eye but still felt like something was in their eye. Dr. observed a scratch but nothing in the eye and prescribed antibiotic.

Showing all 2On 11/10/11, the mechanic was welding overhead on a roof bolter drill slide hose protector. Hot slag rolled onto his left hand and burned his middle finger. The mechanic applied first aid treatment at the time of the incident, but did not report the incident as he felt that it was a minor incident. The burn became infected/abscessed and required medical treatment on 11/22/11.
