Removing sheave from ISC motor-no space to fully lay motor parallel to the ground. The motor/frame were blocked from motion but sheave was hard to remove. EE was using impact wrench on sheave when the tapered bushing let loose & the wrench, sheave & EE's left hand were pulled approx 16" downward. The wrench & sheave pinched the left pinky finger b/t the sheave & crusher structure.

Employee was cleaning material from the second floor of the screen house. A metal plate approx. 6"x 6" had been pushed back and hidden on a ledge of the chute beside the employee but shifted due to the vibration of the plant. The metal plate fell off the ledge, hitting employee's left forearm and resulting in a laceration with three stitches.
