Showing all 4Employee was trimming slate chips using Trimmer #1. She had a chip in the trimmer when the blade got stuck on the left side of the machine in the rock. She tried pushing the rock in, and the blade let loose and cut her finger. She had her fingers on top of the slate which is an unsafe act. Finger are always to be underneath the slate chip.
Employee operating the splitter. He was moving it and the splitter hit an uneven area and dropped and rocked and shook employee around in his seat.
Employee was manually spinning the crushing drum at the Crushing Plant when his foot slipped and got hung between the crushing drum and wear plates. His right foot may be fractured, but is definitely sprained and has soft tissue injury.
