IE put a roof bolt in the top and when IE went to put the bolt in the chuck of the bolter the plate on the bolt slid down the roof bolt and cut EE's forearm. IE received 4 sutures.
Employee hit head in the cab of the dozer when the coal gave way by the feeder causing the dozer to go into the feeder. Employee received sutures.
IE tried lowering a shield that was metal bound with another shield. IE was standing on the feet of the shield and when the shield broke free it bounced and IE hit IE's head off the top of the shield hurting back. Accident Report was not filled out until 3-14-2022. IE worked from the date of the accident until 3-14-22.
