Employee thought the roof bolt was in the chuck and when EE raised the boom the bolt popped out and the roof bolt plate cut EE's arm.

Drill steel popped out of chuck and hit EE in the left thumb. EE had to get 8 sutures.
Employee was running a miner and cutting when a piece of metal broke off a bit and flew back hitting the operator on the right hand.
While trimming top with the continuous miner, the operator was struck in front of the right ear by a small piece of flying metal. The small piece of metal had to be removed and sutured.
Employee pinched left ring finger between roof bolt pod and the winch causing a cut and fracture to the end of the finger.
