Grouters were in the process of pumping water into the 1st hole in preparation of cement grouting when the packer being pressurized was ejected from the face resulting in an injury to one of the grouters.
At approximately 4:30 pm August 30th, 2019; A piece of steel was being cut out of the chute with a sheave rope attached to control the downward motion when the piece was cut free. When the piece came free the rope tensioned and kicked toward an employee who was standing too close to the "v" of the rope striking EE's arm and face.
EE was drilling some metal when the drill hung up causing metal to spin and wrap around bit. Metal lacerated EE's left index finger.
After welding, miner felt something in his eye. Medical exam revealed a metal fragment imbedded in eye.
