While using a grinder to cut a piece of metal guarding, the grinder kicked back and struck EE's left index finger causing a laceration. The lacerations was medically closed with 4 stitches. Employee did not miss any scheduled work days
While drilling a hole to install a 20ft roof bolt, the top steel got stuck in the hole, while retrieving the steel out of the roof, it inadvertently fell pinching their right pinky finger between two steels, causing a laceration and fracture of the tip of their finger
Fractured Cap Rock fell from the open space between Long Wall Shield Canopy Tips and the Coal Face. The rock fell at an angle across the Shearer Ranging Arm and forced the Joy Global LCM assisting with technical service into an encumbered position by pinning their foot.
Employee was bolting when a piece of the rib slid off the ventilation tube and struck EE's neck and left shoulder. Employee was diagnosed with neck contusions and a possible concussion. EE was taken off of work the same day 5/19/26.
EE was operating the headgate side of the shearer, while snaking back into the coal block employee was standing 5 shields away when EE was struck by a piece of coal measuring 3"x4"x1" thick resulting in a fracture of the Maxillary sinus.
EE was pulling shields in, one advanced striking employees feet resulting in 2 broken toes and 2 broken metatarsals.
While employee was center bolting in the #3 entry of the 15L Tailgate Section, the center bolter lost suction to the drill head. While employee was feel for suction with EE's left hand, EE inadvertently drop a drill steel on EE's left index finger. This caused a laceration and a fracture to EE's finger. The employee was subsequently taken off of work.
IE was assisting placing new floor section into a vessel. The crew was in the process of lowering it in with an air hoist. The floor plate shifted and the employee's ankle/foot was wedged between the floor section and the feed impact plate. Injury was not the result of a training or compliance issue.
An employee was emptying a muck bucket that was filled with water to prepare it for a man trip. During this activity, the bucket started to rotate and dump the water towards the employee. The employee stepped in to stop the rotating of the bucket with EE's right hand and caught EE's middle finger between the bail stop and bail. The employees received 5 sutures, and small fractures.
Injured was prepping to drop roller and the grinder came out of their grip from kickback when cutting.
Employee was bolting on the side of the miner in 7 South left section. As EE was pushing the bolt into the hole, the expander caught on the hole causing the bolt to bend. EE's hand was pinched between the bolt and the strap jack. Employee received 4 stitches and was treated and released, no lost time.
While bolting off of the miner in 5K CM section, a piece of rock fell while the employee was switching out the drill steels, pinching EE's hand between the drill steel and the rock. This caused a laceration that required medical attention. Employee missed scheduled days worked as a result.
Employee was a bolter helper that was installing roof bolts, Operator lowered the bolter head and struck the employees hand between a drill steel and the bolter head. This caused a laceration that stitches were used to close the laceration.
EE was cutting a 2'x6'x10' piece of wood in half. EE was cutting with a circular saw. EE's co-worker was holding onto one side of board while EE was holding other side. EE had almost cut completely through the wood. The board dropped and EE's left hand slid forward into the running saw blade. EE has lacerations middle and ring finger and EE severed EE's pinky finger.
Employee was walking by miner after placing crib under the cat. Inby last rib strap, the rib rolled out and broke into pieces, striking them on the right side of the face. This caused a laceration to right ear/head and contusion to face/jaw. Employee received sutures and fractures to the jaw/head. There were rock rib conditions and miner cutting below btm levels unevenly.
The accident happened because roof plate and pie pans were picked up by the magnet and employee was working under the magnet and someone was checking the power and turned off the power to the magnet the items on the magnet let loose and fell on top of the employee. A roof plate struck employee in the back of head.
Employee was installing a T3 strap against the roof from the miner in the 1B CM section. Employee was resting the strap on employee's shoulder during installation. The strap slipped off employee's shoulder and hit the top of employee's right hand causing a laceration behind the index finger. Employee received 5 stitches.
EE placing crib block along the side of head gate gearbox. The shield hydraulic was activated causing the shield to push against the cribbing. The EE's right thumb was pinched/smashed between cribbing and gearbox.
The employee was using a chop saw to cut collar pipe. When employee raised the blade after making the cut the guard snagged employee's glove pulling right thumb into the blade (at the knuckle).
Employee was attempting to use the scoop winch rope to hook up to the chain of the water buffalo. The hook was tightened up against the frame of the scoop bill. When operator winched out, the rope was bound and accidentally tightened instead causing employee to pinch a finger between the hook and the scoop mast.
Employee was resting left hand on the frame of the Headgate drive when the shields began to push the drive forward, EE's fingers were pinched between the HG drive frame and a cover. This caused a fracture to EE's ring and middle finger on the left hand. Employee did not miss any scheduled work days as a result of this incident.
While rib bolting in 1N TG section, employee swung the bolter towards the rib. The bolt wrench was still in the chuck. It hit a bundle of plates and knocked them off the miner. The bundle hit them in the right forearm resulting in 6 stitches.
Employee was making a volcanize splice. EE was using a set of electrical shears to cut the rubber belt. While EE was using the tool, EE had put down pressure on the tool in which it cut up through the rubber and the metal tip came up and struck EE in the left thumb area below the last knuckle on the palm side.
Employee was installing rib bolts off of the miner when a piece of the rib fell and struck them on the right forearm causing a laceration. Employee received stitches to close the laceration and was taken off of work.
Employee was center bolting in the G5 section, employee was installing 16' foot cable boot into a drilled hole when EE felt a pop in their left wrist. Employee was diagnosed with a fracture on 2/13/24.
While adjusting roof mesh on the miner, Employee had their right hand under the mesh to raise it over the bolter mast and their left hand on the bolter mast. A rock fell from the top and struck their left middle finger. Employee was diagnosed with a fracture to the left middle finger.
Employee was installing 16' rope bolts, 50' inby ribline in the #2 entry in H4 section. While applying up pressure and slowly rotating bolt the stiffener tube bent, during the rotation the bent tube struck the employee's left forearm. Employee was diagnosed with a fracture.
Employee was bolting on the left side of the miner in 2B section. After installing a new roll of roof mesh, the employee moved outby alongside the miner. Then the inby ATRS jacks were dropped, a rock fell from the roof hitting the pan and then hitting the employee on their left leg causing a laceration. They received sutures.
While rib bolting, a rock fell striking employee's hand resulting in laceration and causing them to fall and either striking their lower leg or causing them to fall resulting in a fractured ankle.
Employee was rib bolting when employee pinched left index finger between a bolt wrench and a drill steel. Diagnosed with a hairline fracture and received 5 stitches.
Employee was side bolting on the Joy Miner in the #2 entry of 2 South Left when a rock fell striking the employee's left pinky finger. Employee was diagnosed with a fracture. Employee returned to work for employee's next scheduled shift on 7/31/23. No other part 50.20-5 criteria apply.
While shipping old ram jacks off of the longwall face, the ram jack came out of the pan line and struck the injured employee causing EE to fall. As a result, EE received a laceration to left leg that received medical treatment. 9 Stitches. This employee returned to work the next scheduled shift, no lost time.
Employee was rib bolting off the miner at approximately 24+40 survey station. Upper portion of the rib became dislodged striking the legs of the employee, as a result they fell hitting their head on the miner. The rock measured 58" long x 30" high x 6" thick. EE suffered a hematoma and concussion like symptoms. No compliance issues were found. No other part 50 criteria apply.
Employee was attempting to unhook the winch hook while offloading supplies. The operator accidentally winched in instead of out causing the employee's finger to get caught between the winch hook and scoop bucket causing injury.
Employee was bolting on the miner. The drill steels were plugged, while cleaning steels the drill bit fell off. EE bent down to retrieve drill bit, placed right hand on rib door .At the same time the miner began to backup, causing right ring finger to get caught between rib door and rib. EE experienced crushing type injuries to the tip of right ring finger, resulting in sutures.
Employee was center bolting the #9 entry in 2 South Right @ 59 crosscut and positioning bolter to install left side stitch bolt. Canvas was hung in entry to ventilate face; employee did not realize EE's proximity to rib. EE's left elbow encountered the rib forcing their left wrist into the controls. Employee was diagnosed with fractured left wrist.
While operating the tailgate side of the shear, a rock fell from the roof onto the tailgate drum. A small rock came off the drum and hit the employee on right thumb while holding the remote. They received 5 stitches.
Employee was holding up a pumpable crib bag while another employee was using a hilti gun to secure the bag to the top. When the injured employee slipped, they lost control of the bag and was struck in the nose by the swinging bag. Injury was not the result of a training or compliance issue.
Employee was center bolting and experienced suction problems. Employee was checking suction with left hand when the drill steels in the roof released, striking EE's left thumb. Employee was diagnosed with a fracture; incident was reported to management on 3/15/23. No other part 50 criteria apply.
Employee was caught between the operator's side pan wing and rib. Employee was in the process of checking the bits on the C.M. Employee started the pump motor to raise the head to better see the bits, when pump started the pan wing extended pinning employee between pan and rib. The employee received six sutures to right pinky finger. No other Part 50 criteria apply.
Employee was bolting on the 6 South Left Miner. While installing a 12-foot cable bolt, the expander became stuck. The cable bolt bent and pinched EE's right index finger between the bolt and the bolter mast. This caused a laceration that stitches were used to close. Employee did not miss any scheduled work as a result of the injury.
Employee was bolting on the left side of the miner. Rib bolter was swinging out the rib drill to install rib bolt when the drill encountered the roof bolters drill steel. Dislodging the drill steel from the holder and striking the employee in the right cheek. This action resulted in a fracture to the cheek bone. No compliance issues were found.
Employee was bolting on the left side of a CM Miner. Employee was standing behind the rib protector door, after starting the miner head, a rock flew out over the pan and struck the employee's left cheek resulting in a laceration. The employee received 6 sutures. No compliance issues were found.
While assisting a pan line bolter installing bolts, the injured employee's left pinky finger was caught in a pinch point when the bolter operator dropped the mast down. Injury was not the result of a training or compliance issue.
Employee was bolting on the miner and EE's left hand was caught between the ATRS jack and the frame of the machine. The ATRS pad slipped off a ledge when employee reached for a drill steel. Employee was treated for a fractured little finger and received 3 stitches, release to return to work.
Employee was standing on the #7 shield located in the H1Headgate Longwall face. The shearer was cutting back into the snake, when #8 shield advanced it snagged the spare ram jack that was being stored on #7 shield, triggering the ram jack to strike EE's left lower ankle foot. EE was diagnosed with a hairline fracture of the third metatarsal.

Employee was operating the head gate drum of the shear in 5B LW when a rock came off the drum and hit the employee on left hand. Employee received 3 stitches to left index finger.
Employee was rib bolting in the 4B miner section when EE swung the rib bolter towards the rib, the bolter hit a loose bolt knocking over a stack of roof plates that fell from above the rib bolter controls striking the employee on the left forearm causing a laceration. EE received fifteen sutures.
