Employee reached into bag machine to remove jam when the belt grabbed EE's glove and pulled it into the belt pulley. The injury occurred when the employee reacted by pulling EE's hand out.

Employee was changing v-belts on hummer screen when the counterweight moved. The employee's glove was caught by the v-belt causing a finger to be pinched between the belt and the belt pulley.

Loading 29IN face with #8 powder rig, employee was loading the top right corner of the face. The basket tilt cylinder rod snapped causing the basket to rotate forward freely 90 degrees. causing a laceration on right knee

Employee was drilling with a drill. The stinger fell back and then the auger fell out striking the employee on the finger.

Employee was operating a Fletcher Man Up Bolter at 6L 75 Block. When loading the steel back into the mast the employee had hand on the steel and closed the finger clamps on thumb. Employee fractured the tip of thumb and ruptured the nail. Employee returned to work with restrictions.

While drilling the steel wasn't lining up with the top grabbers. Employee got off of the drill to troubleshoot the issue and noticed the finger had a slight bend, not allowing the steel to be pushed in the correct spot. While walking away the steel fell and hit the operator.

Bypassed a guard, Failed to LOTO, and stuck hand in between rollers on the ABMO machine. These mechanical rollers caused a series of lacerations, abrasions, bruising, and second degree burns. The employee told no supervisor of the incident and put a glove on continuing to work. Claimed was hurt at home. Management became aware of the incident on October 15th.

Employee using a Hammer Jack to break concrete. The Hammer Jack slid off the concrete and the point of the tool fell on their foot behind the steel toe. Employee lifted the Hammer Jack off of their foot and inadvertently activated the trigger, causing the tool to puncture their big toe.

Employee was using a cutting torch to cut bolts out to allow for welding. When the bolt was cut, it fell hitting the employees shirt sleeve, rolling down EE's sleeve between EE's welding glove and right wrist.

Employee was operating a grinder with cut off wheel attachment. Employee was cutting chain and lost control of unit resulting in a laceration. Employee received 4 stitches on back of hand near thumb for a 1" laceration and returned to work with light restrictions.

Employee was assisting a stopper drill operator. The stopper drill slide/tipped toward the employee striking the employee on the back side of the right hand. No fractures, swelling, or bruising was observed. Employee was placed on light duty to allow hand to heal.

Employee was positioning a sling while doing structural work. The crane swung away from employee, pulling torso into structure and pulling arm away, straining left shoulder. This was only a first aid. It became lost time when employee had shoulder surgery on March 15, 2020. I discussed how to file this, Initially thought to be FMLA.

Employee was operating a portable band saw to cut a seized bolt. The employee put too much force on the saw and pushed the blade of the saw into finger. EE cut finger.

Contract shaft miner was putting a new chain on an CS Unitec air chainsaw. The miner did not de-energize the airline connected to the air saw. The miner pulled the throttle lever and right hand was still in contact with the chain. This caused a laceration to the miners right palm area of hand which required 6 stitches.

Employee was installing roof bolts with the #5 Roofbolter and while preparing to install a bolt, the grabber became stuck. He grabbed the bolt to hold it in place so that it didn't fail. When he turned to reach for the lever, the grabber smashed his hand that was holding the bolt. He reached for the control, released the grabber, and lowered himself.

EE was using air wand to clear a can on conveyor line. While blowing out can, his right index finger contacted the raised part of a belt splice that resulted in a laceration to his finger. EE was seen and treated at the E.R. in Grand Saline. TX and released to return to work without restrictions.

Employee was in process of retrieving grease gun when the hose was caught accidentally in gearing where inspection cover/guard had partially vibrated open without his knowledge. The gearing caught the hose and pulled it and the grease gun from his hand. This injury reported as Medical Treatment only.
