Employee was holding a 3/4 inch conduit strap with left fingers and a cordless band-saw in EE's right hand. The band-saw was partially resting on a cardboard box, as EE began to cut the conduit strap the cardboard box shifted causing the band-saw blade to come in contact with EE's left index finger causing a laceration to right index finger requiring 3 stitches.

Showing all 6Employee was using an air lance to poke through a thermocouple port opening on a plugged preheater tower cyclone, when the air was turned on to the air lance, hot material poured out of port opening splashed on employees Nomex hood around the right ear and face area. The heat from the hot material radiated through the Nomex hood and caused burns to both these areas.
Employee was cleaning the preheater tower using the water blaster. During this cleaning a large chunk hit water blaster lance causing it to twist while employee was engaged with gun trigger. The trigger part of the gun also twisted along with employee's left hand ring finger, causing a fracture of tip of left ring finger.
Employee was cleaning the burner pipe on the kiln with an air lance. As the employee was extracting the lance from the port, something fell and made an impact on the lance causing it to flip up and around and hit EE on EE's left wrist causing a hand sprain.
EMPLOYEE WAS CHECKING THE PREHEAT TOWER WHEN AN AIR CANNON DISCHARGED PUTTING HOT AIR ON HIS LEFT ARM AND LEFT SIDE OF FACE. CAUSING BURNS.
Drill bound up and kicked back causing drill to strike head. Causing laceration above right eye.
