Employee was pressing pins at clinker drag when ee strained back while handling the press. While the incident occurred on 03/09/2019 it was treated as a first aid because the employee did not want to pursue further treatment at the time. On 05/14/2019 following a visit to a back specialist, employee was prescribed physical therapy to aid in recovery.

Employee was cutting with a torch on the preheater tower when some hot material fell from where ee was cutting and got caught in shirt sleeve, causing second degree burns to left forearm area.

Worker was spot welding expanded metal to the walls of raw mill duct without the use of a welding hood, resulting in a flash burn to the left eye.

While the employee was using a grinder on some metal, EE felt something go in left eye under the proper PPE. They carried EE and flushed eye, but EE had a corneal abrasion and they gave EE some antibiotics for it.

Employee reported a nauseous smell while cutting pipe in #2 compartment. Results of air testing was negative. Next day, employee stated he wasn't feeling well and asked to leave early. This request is being submitted to terminate Citation No.8820578 and should not be construed as an admission that the matters described herein constitute a reportable accident, injury, or illness.

Employee was welding in the baghouse. During the latter part of his shift he began to have eye discomfort and irritation which was diagnosed as arc flash eye burn.
