EE was on the 4th step of an 8 foot ladder, drilling on a lube reel box to install a buggy whip. The drill bound up causing the ee to lose balance and fall to the ground. EE was seen at the ER, diagnosed with a fracture to the radial bone. EE was released to restricted duty.

EE was opening an air door on a motivator and it "sprung" open unexpectedly, striking the EE on the left shoulder, knocking them to the ground. Initial xrays did not show dislocation, but subsequent follow-up found the shoulder was dislocated. The EE is released to restricted duty.

The haul truck operator was getting loaded by a shovel in the CHOP pit. The operator experienced pain in the back and neck area after the third bucket was placed in the truck. Ee was seen at the ER and subsequent follow-up at Golden Health. The ee was put off work and started in physical therapy.

Employee was positioning a HILTI Hammer-Drill inside the re-bar to be able to drill the next hole for re-bar when EE's left hand made contact with the trigger engaging the drill while right hand was on the bit. EE's glove became entangled in the bit causing dislocated fingers and lacerations on the right hand.

Ee was welding and twisting when ee felt numbness in leg. Ee was diagnosed with sciatica and muscle spasm and released to full duty. Claim reopening requested and approved 03/19/19. Conservative measures failed and ee required medial branch blocks on 11/04/19 making this reportable for treatment.

Ee was cutting core, placed rock under running blade. Saw pulled rock and hand into blade, nicking the left index finger knuckle. Stitches required and ee released to restricted duty.

Miner applying shotcrete to newly excavated portion of the east decline tunnel for initial ground support. Shotcrete nozzle plugged causing the supply hose to jump which caused the miner to lose footing and miner fell striking the ground with the right elbow dislocating it.

Employee energized air hose, the opened end of hose whipped around and the hose fitting struck employee on the head. The hose caused a laceration to the employee's head. Employee was released to full duty.

Ee states that due to operating equipment over rough terrain, the seat bounces and jars EE, causing EE's back to hurt. At follow up appointment on 03/28/16, ee was diagnosed with a cumulative trauma to EE's back causing back pain. The employee put off work pending physical therapy.

Ee was using a grinder and got metal shavings on his coveralls. Air pressure from the tool blew the shavings into his eye. He was using PPE. He was seen at the ER and a small piece of metal was removed from the right eye and he was given prescription antibiotic eye ointment. Released to full duty.

The ee was swinging the shovel to the right, the shovel came to a stop and the counterweight hit the track, jostling the employee in the cab. The ee was seen at the ER, diagnosed with a back strain and put off work for 2 days. On follow up, he was certified off work until the next appointment.

Employee was water blasting the north scrubber fan ductwork on the 2nd floor of the roaster. He was blasting over his head when a piece of scale fell. The scale struck his right hand causing a laceration between his thumb and index finger and into his palm. The wound required 14 stitches to be closed.

While running dozer, the employee backed over a rock, jarring his back. He was seen at the ER, diagnosed with a back strain and put off work for one day. He was unable to return to work on 11/10/12 due to medication, returned to work full duty on 11/11/12.

While hanging cable, ee was attaching bolt to cable when the winch slipped and caught his left middle finger in the winch. He was taken to ER, and subsequently transferred to another hospital for orthopedic surgery to repair the finger. Put off work pending surgical follow-up.

Ee was operating shovel, bucket was on the ground and a small rock hit the windshield breaking the glass and getting glass particles into employee's eyes. Seen at ER, no foreign body in eyes, diagnosed with a corneal abrasion, given prescription eye drops and released to full duty.

Maintenance employee working in the underground surface shop, eyes were exposed to ultraviolet radiation, diagnosed with actinic conjunctivitis and prescribed medication for pain.
