IP was holding the bolt with vice grips while cutting with a grinder when the grinder kicked and caught the top of EE's thumb. IP went to driller and reported injury. All notifications were made, and IP was transported via crew truck to Battle Mountain for evaluation. The IP received 3 Stitches.

Employee was tasked with blend grinding a weld on the bottom of the car body. A foreign body metal shard embedded in the employees left eye.
While the employee was removing nuts from rubber tire dozer cutting edges, the air exhaust from the one-inch impact gun blew dust and other materials towards the employee. The employee felt something in eye but continued to work on 12/19 and 12/20. On 12/21 the eye was red and irritated and the employee reported it to the supervisor.
Employee was removing haul truck tires using a rad gun. When employee began to remove a nut on haul truck tire, the rad gun was not set to remove causing rad gun to tighten and pinch finger between nut and rad gun.
While an employee was being loaded underneath the shovel, the employees neck and back were jarred when the material was loaded into the back of the haul truck.
While attempting to remove the table hammer the employee raised the hydraulic wrench with right hand and reached for the hammer with left hand and the hydraulic wrench slipped out of EE's hand and impacted the left hand.

The pin on the slot wrench broke while making a connection. The driller asked the helper to move the wrench back into place by hand. After doing so the driller placed hand back on the controls to move the hydraulic ram. Then the helper moved hand back onto the wrench as the ram moved, pinching EE's finger.
While torquing lug nuts on the left inside dual on a haul truck, mechanic placed left hand, middle fingertip between the wheel and the rad gun which resulted in a crush injury. The fingertip was amputated at the first knuckle.
While grinding overhead on haul truck, employee received a foreign body to the left eye.
Dozer Operator was pushing berm at dump, when operator turned to reverse, brakes locked up forcing operator to be pushed into seat belt pulling muscle in back.

Employee was driving a rubber tire dozer across a pit floor in muddy, wet conditions. The employee drove through a pothole which jarred his back, resulting in a herniated disc and modified workdays.
While removing wear iron from loader bucket, employee placed hand on top of bolt while removing nut with impact gun. Bolt spun and cut employee's left ring finger with sharp edge on head of bolt. Laceration required stitches.
