While cutting old conduit for removal, about 30 feet of conduit came off the brackets and got caught on the band saw the employee was using and pulled the employee down, which caused a strain/sprain in employee's left groin area. Employee was operating a D-9L Dozer at the raw coal pile when the dozer slid into the feeder causing the employee to strike head against the window of the dozer. EE suffered a laceration to the head that required 5 staples and to the ear that required 7 stitches. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

The rib pinner auger got stuck in the rib while turning a corner. The wrench was put on the rib pinner auger to spin the auger out, when the wrench came out of the chuck and struck the employee in the right eye, resulting in 5 stitches and a fractured orbital bone. Employee was attempting to drill a hole for a six foot resin bolt. When employee attempted to push up the drill steel it sprung out of the bolting pod striking EE's left hand index finger. Compliance with rules/reg., PPE, mining equipment/systems not a factor. Miner proficiency may be a factor.

Employee attempted to reinstall a roof bolt that slipped out of the bolting chuck. Employee was tramming tailpiece on 5W longwall when fitting on whip line broke and sprayed employee with emulsion. Employee was diagnosed with chemical conjunctivitis. Cause of injury was awareness. Compliance with rules/regs,PPE,mining equip/systems not a factor. Miner proficiency may be a factor

Employee was in 6W in the process of handing a roof strap to bolters when the miner oper sumped head back pinching employee's L hand between the rib of #3 entry and the stored roof straps. The accident resulted in the amputation of the left middle ring finger at the mid knuckle. Cause of injury was awareness. No other pt 50 criteria applies.

Employee was cutting face chain on the 4WLW face at approx 3 shield when the chain let loose pulling ee into the top deck of the panline causing lacerations to ee's head, a fracture to ee's nasal cavity and burns to ee's right arm. It was determined the chain was still tensioned and procedures have been put in place to prevent re-occurance.

Contract employee was air arcing with a welding shield on and wrap-around safety glasses on and something got behind everything and went into EE's eye. Cause of this incident was possible face positioning. Compliance with rules/regs., PPE, mining equip/systems and/or behavior was not a factor.

Employee on 4E LW at 158 shield, the shearer was cutting through gas well, pan was pulled back to hit well from TG. The shearer was mining at #168 and the infrared program was initiated, pulling 158 shield striking EE pining leg against pan fracturing tibia and femur. No compliance issues It didn't involve job skills proficiency training, attitude PPE or protective devices

Employee was assisting install hydraulic hose on shield on the 4E longwall. When they finished they put pressure when another hose flew striking employee on left leg resulting in fracture to left tibia. There were no compliance issues. It didn't involve job skills proficiency training or attitude PPE or protective devices no equipment involved.

Employee was drilling with a cobra drill on 7-38xc roof fall when the drill steel became stuck in the rock. While pulling on drill steel employee felt pain in back. No other part 50 criteria apply.
