The employee stated that they were pushing a bolt in a hole when a small piece of rock fell, striking the employee's tooth. The employee sought dental attention and had the tooth repaired.
The employee was in the bolter station when the miner advanced a piece of rock that got caught between the back support and the rib, causing the jack to yield due to the bolter back stop not fully extended, causing the employee to sprain their left ankle. The employee was taken off work on 4/30/2025. This is being reported as an LTA.
Employee was assisting a roof bolter operator backing down the entry when the cable fell off the tray. The employee repositioned the cable and, while walking back to the front, was struck by the ATRS pad in the shoulder.
Employee was tightening allthread on steel set and had hand on wrench. The impact gun tightened on bolt and the wrench accidentally swung around and caught finger between wrench and steel set causing injury.
Employee stated EE was installing waterline clips and when EE tried to swing the bolter head a piece drill steel became fouled and struck EE in the forehead.
Employee stated they were trying to align the T- Channel roof strap and smashed their thumb between drill mast and strap.
A piece of coal fell and struck employee left shoulder. Employee missed their next shift.
Employee stated was trying to connect two pieces of drill steel together when EE caught left index between steel and drill mast.
Employee stated EE was in the process of hooking up a winch to pull belt. When EE raised up a highline hook caught EE's ear cutting EE's ear. Employee received 8 stitches in EE's left ear.
Employee stated they were uncoupling a dust and the hose still had pressure on it and came off the end of pipe causing them to stumble backwards and strain their back. Employee started missing work on 4/20/23
Employee stated they were scaling down a piece of rock and it hit the canopy and slid off the canopy striking them in the upper part of their body.
Employee stated a piece of rock fell from the top striking them in the left shoulder and head. Employee started missing work on 12/28/2022.
The employee stated they were struck by a strap plate causing a puncture. Employee sought medical treatment and was given sutures. This incident is being reported as a medical only.
Employee stated they were installing a bolt when a piece of rock fell out of mine roof striking their right calf and foot.
Employee stated they twisted to hit the levers and felt a pop in their back.
Employee was bolting on right side of miner when a piece of rock measuring 3 feet by 3.5 feet struck them on the head/neck.
Employee stated: a piece of drill steel came out of top striking EE's hand causing a puncture to EE's left hand.
Employee stated: was using a grinder R2 bin when a piece of metal struck EE in the eye.
Employee stated they were installing roof support when a rock fell out striking their right foot.
Employee was rock dusting along the beltline. As employee was backing up employee tripped over a loop of dust hose and fell backwards on back. Employee sustained a bruised and strained back.
Employee was operating a pneumatic banding gun that jammed. As EE attempted to remove the air line to clear the jam EE dropped the gun. When EE caught the gun EE inadvertently shot self in the right index finger cutting the very tip of EE's finger off.
Employee was removing drill steels from the roof when one of the steels fell from the drill hole and smashed employee's left middle finger between two drill steels. The employee suffered a lacerated left middle finger.
Employee was advancing miner cable slack from 2 - 3 xc to #3 entry. EE hooked the cable to a shuttle car in #3 entry to pull the loop of slack thru three cable handlers around the corner. Employee put themselves in bad body position. The cable pulled tight and came flying out of cable handlers and struck the employee in the shoulder knocking employee into the rib.
Employee was cutting a bolt off a piece of belt structure on the slope belt. Employee was using a electric grinder. While cutting the bolt the grinder wheel broke apart and struck the employee in the left hand causing a laceration that required several sutures.
Employee was standing on #5 shield as the shearer cut out on the longwall. A piece of slate came flying off the cutting drum and struck the employee in the left knee. Employee was taken off work on 4/15/2020.
Employee was operating the tail drum of the Longwall shearer when a piece of rock came off the drum and struck the injured in the right index finger causing a laceration that required five sutures.
Employee was hosing shields as shearer was cutting to the tail. The shield the EE was hosing started to come in and the EE tried to step up on the pontoon and foot slipped off and got caught between the pontoon and the relay bar. The EE was five shields outside of the safety zone. The shearer operator hit the E Stop when operator saw what was happening.
Employee was installing a roof bolt when EE bent the bolt. As EE tried to straighten the bolt EE inadvertently hit the wrong lever and pinched right hand between the roof bolt plate and the mast of the miner that caused a laceration to EE's right hand. The wound required sutures.
Employee was installing a roof bolt off the miner side bolter when the collar of the bolt caught on the roof strap causing the bolt to bend and smash the employee's right ring finger against the bolter mast causing a fracture and laceration.
Employee was attempting to move a vent tube along side the fullface miner when the loading machine hit the tube and pinched the employees right index finger between the tube and the rib protector causing a laceration.
Employee was bolting with another EE using a single boom bolter. While re positioning the bolter for the next bolt the operator dropped the ATRS and it hit a drill steel which the helper was holding and pushed the drill steel down onto the Injured EE's right foot.
Employee was releasing a boat winch on the LW face during the screening process when the handle struck in the right wrist.
Employee was installing a buggy snub anchor when the employee pinched left middle finger between the bolt plate and the bolt causing a laceration that required sutures.
Employee was raising mast on bolter of the continuous Miner while checking the vacuum pressure. When employee lowered the mast they cut the tip of the right middle finger.
EE was loading a double boom bolter on a dolly when EE pinched right index finger between a metal guard and the frame of the bolter causing a laceration that required seven sutures.
On walkway side of beltline bolter helper, was taking out drill steel from roof, the steel was stuck so the bolter ran up the stab jack to relieve pressure on the steel. The stab jack smashed the top of the employees right foot between the jack and the frame of the bolter.
EE was changing out drill steels on a horizontal degas drill when EE inadvertently placed left hand between the pipe clamp and the drill steel. The hydraulic pipe clamp closed on the EE's left hand breaking the ring and middle fingers and amputating the tip of the middle finger.
EE was helping the operator bolt on a double boom bolter. EE was removing drill steels as operator lowered bolter head. The EE inadvertently put foot under the drill head as the operator dropped the head to the ground striking the EE on the top of the foot. The EE worked till 8/26/2017 when EE was taken off work for a contusion of left foot.
Employee was cutting with a torch when flash back occurred, burning employee's left hand.
The EE was attaching a grounding lug to a starter panel with a tech screw. In the process of attaching it, the tech screw kicked causing the EE left forearm to turn into the edge of the panel. This resulted in a laceration requiring 10 sutures.
