Showing all 10Employee stated that while they were drilling the outside bolt hole with the starter steel, a piece of rock that measure 6"x 8"x 1/2" fell out and struck them on the right forearm. This resulted in a laceration.
Removed lagging from drive roller and was buffering and cleaning up drum to install new lagging grinder got caught and kicked back resulting in laceration to pinky finger on right hand and small cut on ring finger .

EE was drilling a hole to set a cable bolt when placed hand on the drill steel. EE's glove got pinched between the joints of steel and wrapped EE's right arm around the steel.
Employee stated that they were bolting top when a rock fell out and hit the canopy. Once the rock hit the canopy, it broke and then fell back in the drill station and struck them in the left forearm.
EE stated that EE drilled EE's 3 ft. hole and went to push EE's stack steel up the hole when the insert broke off causing the steel to miss the hole. The steel hit the top and then bowed out, broke, and hit EE in the right side of the face.
IE was bolting on the operators. As they were bringing down the drill pot, left foot slipped under the drill pot and smashed foot.
EE was drilling a 12' hole, so that EE could install a 12' cable bolt. EE stated "I had just put the last piece of stack steel in the drill pot to finish drilling the hole," at which time EE pushed the joystick / rotation, at which time the drill pot caught EE's glove, wrapping EE's arm around the steel once. EE walked self back around the steel.
