The bulk loader proceeded to clear the air slide and opened the clay belly to address a clog using an air hose. Upon opening the air valve, cement was expelled in the opposite direction, resulting in cement entering THEIR left eye. Eye wash station used, but followed up with eye doctor.

Showing all 7While making wooden storage boxes EE attempted to drive a 4 inch screw through a 2x4 using an unstable support, the screw pierced through the wood, EE's shoe, and into EE's right foot at the base of the middle toe.
Employee was using a 4" Dewalt Grinder to clean up a weld. The grinding disk broke apart and a piece of the the disk lacerated the back of left hand behind the middle finger. Employee was transported via company vehcile to an urgent care facility and received sutures
Worker lacerated skin on four fingers using a saw cutting wooden wedges
Were removing interference fitted sprocket while installing bucket elevator sprocket under pressure with porta power ram, the bolt to the puller broke while employee was in front and struck employee left outer bicep.
Was breaking up hardened material with jackhammer, jackhammer broke causing loose material to fall and EE slipped on loose material. EMTs were called and EE was released from ER that night and followed up with DR who put them on restrictions:NO heavy lifting, pushing, pulling, climbing, overhead reaching, only sedentary work. Will be reevaluated 3/17/21.
Contractor was repair a coupling when the machine turned back on and caught EE's hand.
