Showing all 15Employee was using 20k hydro-blast flex lance to clean inside of 6 inch pipe. The employee pulled the lance out of the pipe under pressure, allowing the tip of the lance to come back and strike employee in the right forearm, causing a laceration to the arm.
EMPLOYEE WAS DOING HOUSEKEEPING AFTER JOB WAS COMPLETE AND EE WAS MOVING A VALVE USING THE ELECTRIC HOIST TO REMOVE THE BEARING MOUNT ASSEMBLY. WHILE USING THE PULL CHAIN TO MOVE THE HOIST TOWARDS THE DOOR, THE HOOK SLIPPED CAUSING THE TOP OF THE MOUNT TO FALL TO GROUNG AND STRIKING THE LEFT FOOT.
Employee was descaling 2/3 Hopper when a piece of scale fell and hit the gun. The gun in turn struck the employee in the face area
Maintenance GR personnel was on top of Tray # 3 assisting with the disconnect of torque mechanism. EE began removing the bolts that secure the torque mechanism. Upon the removal of the last bolt, the torque arm suddenly released and pushed the torque mechanism and mounting plate. The force of the release caused mounting plate to strike employee in the leg.
Employee was welding a bracket on a 24 inch valve when eyes was exposed to a welding flash due to the failure of auto lens.
While shearing a small piece of 1/8" metal plate, the employee got 1 finger on each hand smashed = (2 fingers). EE was taken to Medical for evaluation and then off Site for further evaluation.
Operator was using the swivel vice at the control room lean-to putting together a short entry nozzle. While tightening the 90 piping it slipped and hit EE in the ear, resulting in a small cut.
While grinding a banana wedge on a bench grinder, the wedge was pulled into the grinder causing right pinky finger and ring finger to be caught in between the grinder wheel and tool rest.
Employee while working on a dozer pushing material slid into a cavity causing employee's head to move side to side rapidly causing a strain to neck
Operators were beginning to work on installing steam and air rig on TK61 for caustic cleaning. While in process of installing steam hose operator was burned by steam.
IW was connecting rigging to an excavator bucket when EE's left foot was pinched under the bucket resulting in partial amputation of left big toe. Bucket moved due to listing of derrick barge when crane on the barge swung in direction of dock. Excavator was on derrick barge with arm extended over the dock, IW was on dock rigging the load when incident occurred.
Employee was a welding helper cutting and welding stainless steel. Employee worked a double shift and noted a problem on the drive home. Employee reported the eye incident the following day. Employee was taken off site for initial visit on 10/17/2019 and the following day was seen by an eye specialist on 10/18/2019.
Employee was removing scales with a chipping hammer when a piece of debris was knocked off and punctured the glove on employee's left hand. When the glove was removed there was a fine point on left thumb.
The employee was changing a control valve on # 4 boot pump. EE has the controlroom operator stroke the valve and while stroking EE felt the glove on right hand being pulled into the shaft.
